Over the years, the number of pediatric patients undergoing surgeries are increasing steadily. The types of surgery vary between elective to emergency with involvement of multidisciplinary teams. The development of day care surgery unit is expanding where the patients will only come to the hospital on the day of surgery and discharge home after such as satisfactory parameters achieved, minimal to no pain, minimal to no bleeding from surgical site and able to tolerate fluids. Hospitalization and surgery could contribute to significant psychological disturbance to the children. These issues are not being addressed as children have difficulty in conveying their problems and fear. They do however express it through negative behavioral changes.

Losartan and other modulators of the Renin-Angiotensin System (RAS) have been used for their anti-fibrotic effects by non-surgeons for decades where suppression of maladaptive scar deposition is helpful, such as heart failure and chronic kidney disease. Only recently have orthopaedic surgeons begun to explore whether these medications might reduce or prevent post-operative joint stiffness. The relationship between orthopaedic surgeons and fibrosis is a more complicated.

Surgical complications remain an unfortunate inevitability of surgical practice. When adverse events arise, orthopaedic surgeons must be prepared to navigate the complex medical, ethical, and legal dimensions through a multifaceted response. Prompt communication and collaboration with the risk management team, along with proper documentation, are essential.

In addition to elevations in serum creatinine, kidney dysfunction encompasses inadequate maintenance of sodium and volume homeostasis, retention of uremic solutes, and disrupted endocrine functions. Hemodynamic derangements and maladaptive neurohormonal upregulations contribute to fluctuations in kidney indices and electrolytes that may recover with guideline-directed medical therapy.

Background: A trusting partnership between patient and surgeon lies at the core of a successful therapeutic relationship. However, there are conditions that can jeopardize this partnership and create an ethical dilemma for the surgeon, who is guided by a moral obligation to care for the patient without causing unnecessary harm. We present a discussion of several pathways to achieve ethical resolution to therapeutic relationships that have grown untenable with a focus on patients with comorbid psychiatric illness.

Despite the progress made in medical therapies for treating pulmonary hypertension (PH), a subset of patients remain susceptible to developing a maladaptive right ventricular phenotype. The effective management of end-stage PH presents substantial challenges, necessitating a multidisciplinary approach and early identification of patients prone to acute decompensation. Identifying potential transplant candidates and assessing the feasibility of such a procedure are pivotal tasks that should be undertaken early in the treatment algorithm.
