Background: Postoperative hypoparathyroidism is the main reason for outpatient follow-up and long-term oral calcium and calcitriol treatment. Our study investigated the influencing factors and powerful predictors of short-term postoperative parathyroid function recovery.

Methods: Logistic regression was used to compare the clinicopathological characteristics; surgical details; and serum calcium (Ca), magnesium (Mg), and phosphorus (P) concentrations of patients. A receiver operating characteristic (ROC) curve was used to analyze the predictors of normal parathyroid hormone (PTH).

Results: Among the 111 patients with PTH < 10 pg/mL on the first postoperative day, most patients experienced a return to normal PTH (PTH > 15 pg/mL) within 30 days postoperatively. Univariate analysis showed that Pod (postoperative day) 1 PTH, Pod3 PTH, Pod7 Ca, Pod7 Mg, and Pod7 P (P < 0.05) were associated with parathyroid function recovery to normal on the seventh postoperative day. Multivariate logistic regression analysis revealed the following independent risk factors for normal PTH levels at Pod7 after thyroidectomy: Pod3 PTH (P = 0.038), Pod1 PTH (P = 0.056), Pod7 Mg (P = 0.001), Pod7 P (P = 0.020), and the number of parathyroid glands in situ intraoperatively. The combined sensitivity of serum magnesium concentration and phosphorus concentration to predict parathyroid function recover to normal on the seventh postoperative day was 82.76%, with a sensitivity of 76.83%.

Conclusion: Serum magnesium, phosphorus and PTH concentrations are important influencing factors and effective predictors of short-term postoperative parathyroid function recovery to normal. Serum ion is an effective auxiliary diagnostic method for hypoparathyroidism after thyroidectomy.

Background/aim: Platelets not only participate in physiological hemostasis but also play a major role in liver ischemia-reperfusion injury, liver damage, tissue repair, and liver regeneration. A decrease in platelet count can lead to spontaneous bleeding, infection, and other complications that can seriously impact patient prognosis. Thrombocytopenia has been associated with increased complications after partial hepatectomy, although the effects of thrombocytopenia on patient outcomes remain unclear.

Neuropathic pain is a pervasive medical challenge currently lacking effective treatment options. Molecular changes at the site of peripheral nerve injury contribute to both peripheral and central sensitization, critical components of neuropathic pain. This study explores the role of the G-protein-coupled bile acid receptor (GPBAR1 or TGR5) in the peripheral mechanisms underlying neuropathic pain induced by partial sciatic nerve ligation in male mice.

Causes and predictors of unplanned reoperations within 30 days post laparoscopic pancreaticoduodenectomy: a comprehensive analysis.

Front Oncol

Department of Hepatobiliary and Pancreatic Surgery, Ningbo Medical Center Lihuili Hospital, Ningbo University, Ningbo, Zhejiang, China.

Article Synopsis
  • The study aimed to identify risk factors and causes for unplanned reoperations within 30 days following laparoscopic pancreaticoduodenectomy (LPD), reviewing data from 311 patients at a Chinese medical center.
  • Out of these patients, 7.4% (23 individuals) underwent unplanned reoperations primarily due to postoperative bleeding, with other causes like anastomotic leakage and infections also noted.
  • Key independent risk factors for these reoperations included low pre-operative serum prealbumin, high CRP levels on post-operative day 7, vascular reconstruction during surgery, soft pancreatic texture, and a narrow main pancreatic duct diameter.

Article Synopsis
  • Colorectal surgery for cancer carries a significant risk of complications like anastomotic leakage, making it important to predict this risk early for personalized treatment plans and reduced hospitalization times.* -
  • The study evaluated the neutrophil-to-lymphocyte ratio (NLR) in 245 colorectal surgery patients, finding that an NLR of ≥ 7.4 on postoperative day 1 can reliably identify low-risk patients for leakage, while an NLR of ≥ 6.5 on postoperative day 4 is useful for identifying high-risk patients.* -
  • The results suggest that using NLR measurements from day 1 can help doctors focus on patients who may need closer monitoring for leakage, improving patient management and outcomes

Objective: The aim of this study was to observe the recovery of liver function in patients with early allograft dysfunction (EAD) at one month postoperatively and explore whether early postoperative ultrasonic hemodynamic parameters can predict this outcome.

Methods: Patients with EAD postoperatively, who underwent orthotopic liver transplantation (OLT) in our hospital from December 2012 to June 2021, were retrospectively collected. Based on the recovery of liver function within one month, patients were divided into two groups: the recovery group (FR-1M) and the non-recovery group (Non-FR-1M).
