Right ventricular failure (RVF) is a serious adverse event after left ventricular assist device (LVAD) implantation but difficult to be characterized. This study aimed to visualize the dynamic circulatory equilibrium of acute RVF after LVAD implantation using a new four-quadrant diagram constructed by ) cardiac function with central venous pressure (CVP) and cardiac index (CI) axes, ) arterial vascular resistance with CI and mean blood pressure (mBP) axes, ) pressure-diuretic function with mBP and net urinary sodium output (net U-Na) axes, and ) venous compliance with net U-Na and CVP axes. Twenty LVAD patients were stratified into two groups, (≤10 days) and (>10 days), according to duration of postoperative inotropic support. The preoperative equilibrium loops were small in both groups. In the early postoperative phase, the loop in became dramatically enlarged to the left and upward, indicating increased CVP and CI by LVAD support. In , however, augmentation of CI was smaller despite similarly increased CVP, and net U-Na was decreased despite increased mBP. In the late postoperative phase, the equilibrium loop in recovered as similar to that seen in . Thus, acute RVF, as shown in , was characterized by the shape of the loop constructed by marked increased CVP, a relatively small increase in CI, and concomitant impairment of pressure natriuresis. In conclusion, the novel four-quadrant presentation of systemic circulatory equilibrium provides clear visualization of RVF after LVAD implantation, thus serving as a useful guide for prompt and optimal management. Systemic circulatory dynamics are regulated by various negative feedback systems, including cardiac, arterial, venous, and renal functions, as well as autonomic nervous systems. The present novel four-quadrant presentation of their functions allows clear visualization of dynamic organ-to-organ interactions that can lead to a new circulatory equilibrium after therapeutic intervention. This new system physiological framework can serve as a useful guide for prompt and optimal management of circulatory malfunction.

Positive end-expiratory pressure (PEEP) improves respiratory conditions. However, the complex interaction between PEEP and hemodynamics in heart failure patients makes it challenging to determine appropriate PEEP settings. In this study, we developed a framework for the impact of PEEP on hemodynamics considering cardiac function, by integrating the impact of PEEP in the generalized circulatory equilibrium framework, and validated the framework by assessing its ability to accurately predict PEEP-induced hemodynamics.

Acute cerebral ischemia alters brain network connectivity, leading to notable increases in both anatomical and functional connectivity while observing a reduction in metabolic connectivity. However, alterations of the cerebral blood flow (CBF) based functional connectivity remain unclear. We collected continuous CBF images using laser speckle contrast imaging (LSCI) technology to monitor ischemic occlusion-reperfusion progression through occlusion of the left carotid artery.

Background: Patients with renal hypoplasia are often associated with congenital heart disease (CHD). During the perioperative period of kidney transplantation (KTx), sufficient circulation volume is required to maintain renal blood flow in the donor kidney. However, little is known about the indication and management of KTx in patients with CHD who require precise hemodynamic assessment during transplantation.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) benefits patients with cardiogenic shock (CS) but can increase left ventricular afterload and exacerbate pulmonary edema. Adding a percutaneous left ventricular assist device (LVAD) to VA-ECMO can optimize the hemodynamics. Because managing VA-ECMO and LVAD simultaneously is complex and labor-intensive, we developed a closed-loop automated control system for VA-ECMO and LVAD.

Patients with fatty acid oxidation disorders (FAODs) experience muscle symptoms due to impaired ATP metabolism and the toxicity of accumulated mitochondrial FAO substrates or intermediates, especially during catabolic states. A major issue is the absence of specific and sensible biomarkers to evaluate metabolic equilibrium. The relationship between cardiac output (Q) and oxygen consumption (VO) during incremental exercise (dQ/dVO) provides an indirect surrogate of mitochondrial function.
