Article Abstract

Surgical treatment of brainstem cavernoma is controversial. With modern surgical technique, safe and complete removal of selected brainstem cavernoma is possible. The choice of optimal corridor must consider various factors, including eloquent structures of the brainstem, cavernoma shape and location, as well as the presence of an exophytic portion to the lesion. The endoscopic endonasal approach, with its refinement through the last decade, could offer an optimal corridor for the removal of ventral brainstem cavernomas. This video exemplifies the key surgical steps, advantages of the technique, and relevant anatomy for the endoscopic endonasal removal of a ventrally exophytic pontine cavernoma.

We report a case and follow-up of an adult male with intracranial yolk sac tumor (YST). Background: Brainstem cavernous malformations (BCM) constitute one of the most controversial and challenging neurological pathologies: both natural course and surgical manipulation can lead to severe neurological symptoms by direct compression or following hemorrhage of this highly eloquent brain region.

Methods: The vascular section of the prospectively collected neurosurgical database of our center (2011-2023) was retrospectively reviewed, seeking all patients operated on for a sporadic BCM. Clinical, radiological and surgical data, operative records and videos were analyzed by independent reviewers with the assistance of a dedicated neuropsychologist, blinded to the hospital course.
