[The concept of paedophilia. Part 1 : Historical background and nosology].

Paedophilia is a paraphilia characterised by behaviour, drives or sexual fantasies that are intense and recurring and that involve children, in a general sense. This sexual deviation is often linked to personal distress, to a significant alteration of personal, family, professional and social life, in a general sense. In our modern Western societies, paedophilia and the perversions associated with it are punishable by law. Paedophilia is a concept that is very ancient, complex and particularly diverse in its many clinical presentations. Its societal impact is very important and contrasts with the real knowledge of this issue among non-specialists, or even among specialists. Based on expert clinical experience gathered at the courts and on an exhaustive review of current literature on the subject, the authors propose an update of the concept of paedophilia. In the first part of this work, the authors will discuss several aspects such as the historical background and the clinical aspects and nosography of paedophilia. This literature review will be accompanied by short clinical vignettes, based on the authors' clinical experience.

Visual attention plays a central role in current theories of sexual information processing and is key to informing the use of eye-tracking techniques in the study of typical sexual preferences and more recently, in the study of atypical preferences such as pedophilia (prepubescent children) and hebephilia (pubescent children). The aim of this theoretical-empirical review is to connect the concepts of a visual attention-based model of sexual arousal processing with eye movements as indicators of atypical sexual interests, to substantiate the use of eye-tracking as a useful indirect measure of sexual preferences according to sex and age of the stimuli. Implications for research are discussed in terms of recognizing the value, scope and limitations of eye-tracking in the study of pedophilia and other chronophilias in males and females, and the generation of new hypotheses using this type of indirect measure of human sexual response.

In order to treat individuals with pedophilia who are at risk of committing offenses, disclosure of the attraction must first take place. The aim of this study was to understand processes of initial recognition of pedophilic attraction, disclosure, and help-seeking. We conducted a qualitative content analysis of online posts from self-identified individuals with pedophilia, finding four categories: (1) (with emotions including denial, shame, and fear), (2) (typically made to family, friends, or therapists, but also done online in an anonymous way), (3) (ranging from rejection to support), and (4) (including minimization, distortions, despair, resignation, and non-offending/anti-contact commitment).
