Background: To evaluate changes in the effectiveness of phentolamine mesylate in combination with different local anesthetics (LAs) and vasoconstrictors. A prospective randomized double-blind study was conducted with 90 patients divided into three groups, with each group being administered one of three different LAs: lidocaine 2% 1/80,000, articaine 4% 1/200,000, and bupivacaine 0.5% 1/200,000.

Methods: We compared treatments administered to the mandible involving a LA blockade of the inferior alveolar nerve. Results were assessed by evaluating reduction in total duration of anesthesia, self-reported patient comfort using the visual analog pain scale, incidence rates of the most common adverse effects, overall patient satisfaction, and patient feedback.

Results: The differences among the three groups were highly significant (P < 0.001); time under anesthesia was especially reduced for both the lip and tongue with bupivacaine. The following adverse effects were reported: pain at the site of the anesthetic injection (11.1%), headaches (6.7%), tachycardia (1.1%), and heavy bleeding after treatment (3.3%). The patients' feedback and satisfaction ratings were 100% and 98.9%, respectively.

Conclusions: Efficient reversal of LAs is useful in dentistry as it allows patients to return to normal life more readily and avoid common self-injuries sometimes caused by anesthesia. Phentolamine mesylate reduced the duration of anesthesia in the three studied groups, with the highest reduction reported in the bupivacaine group (from 460 min to 230 min for the lip and 270 min for the tongue [P < 0.001]).

Pheochromocytoma and paragangliomas (PPGLs) crises can be triggered by various factors, including norepinephrine reuptake inhibitors used to treat attention deficit hyperactivity disorder (ADHD), which worsen symptoms in patients with PPGLs. Therefore, attention should be paid to the potential for serious adverse reactions in patients with PPGLs taking ADHD medications. A 21-year-old man presented to the emergency department with acute onset of severe respiratory and circulatory failure after initiating atomoxetine treatment.

Article Synopsis
  • The melanocortin system plays a crucial role in controlling energy balance and weight regulation, with recent findings indicating its effects on peripheral tissues like skeletal muscle, independent of the brain or SNS.
  • Infusing α-melanocyte-stimulating hormone (αMSH) directly into the femoral artery significantly increased heat production in skeletal muscle, while not affecting food intake, demonstrating its local action in energy expenditure.
  • The study also revealed that αMSH infusion enhanced mitochondrial function, increased glucose uptake in skeletal muscle, and showed that blocking SNS with certain drugs did not impact αMSH's effects, highlighting its unique role in energy regulation.

Background: Dental anesthesia administration often triggers unpleasant sensations, particularly needle injection-related pain, which can evoke fear among patients, especially in the pediatric population. Vibration and low-level laser therapy (LLLT) have been extensively studied as potential methods for alleviating pain. Additionally, phentolamine mesylate (PM) has shown promise in reducing the duration of anesthesia.

The basolateral amygdala (BLA) contains adrenergic receptors, which are known to be involved in stress, anxiety, and memory. The objective of this study was to explore whether inhibition of α-adrenergic receptors (by phentolamine, an α-adrenergic receptor antagonist) in the BLA can reduce foot-shock stress-induced anxiety-like behavior, memory deficits, and long-term potentiation (LTP) deficits within the CA1 region of the rat hippocampus. Forty male Wistar rats were assigned to the intact, control, stress (Str), Phent (phentolamine), and Phent + Str groups.

Article Synopsis
  • This study focuses on improving cartilage regeneration using stem cells by addressing issues like off-target differentiation and hypertrophic tendencies.
  • Researchers created a system to monitor chondrogenesis and hypertrophy in real-time using stem cells with specific genetic markers and tested over 2,000 FDA-approved drugs.
  • They discovered that the drug phentolamine promoted cartilage growth and avoided degeneration, suggesting the α2-AR/SLPI pathway as a potential target for effective cartilage regeneration.
