Background: This study compared short- and mid-term outcomes of hemihepatectomy (HH) and pancreatoduodenectomy (PD) in patients with extrahepatic cholangiocarcinoma, focusing on surgical outcomes, body composition, and nutritional status.
Method: A retrospective review was conducted to assess short-term outcomes, including operative time, blood loss, complications, and mortality. Body composition and nutritional parameters were analyzed preoperatively and 1 year postoperatively.

Background: Although laparoscopic hemihepatectomy has gained prominence, one of the critical challenges in this procedure is the approach to the middle hepatic vein (MHV). The MHV, which runs in the midplane of the liver, is situated above the hilar plate and serves as an anatomical landmark in hemihepatectomy. We have introduced dorsal approach to the MHV from the hilar plate in laparoscopic hemihepatectomy under the laparoscopic caudo-dorsal view.

Background: Most patients with hepatocellular carcinoma (HCC) have lost the opportunity for direct surgery at the time of diagnosis. Transarterial chemoembolization (TACE) combined with immune checkpoint inhibitors or tyrosine kinase inhibitors (TKI) can partially transform some unresectable HCC and improve the prognosis effectively. However, based on the promising prospects of combined targeted and immunotherapy for the effective treatment of HCC, the positive role of palliative surgery in the conversion treatment of advanced HCC urgently needs further intensive re-assessment.

A 77-year-old man underwent a right hemihepatectomy for hepatocellular carcinoma in 2002. The patient remained alive without recurrence for 17 years after surgery. In September 2019, he was diagnosed with a tumor in the lower lobe of the right lung by follow-up CT.
