Duodenal lymphocytosis (DL) is a condition characterized by enhanced infiltration of intraepithelial lymphocytes (IELs) in the duodenal mucosa, and it can be linked to both gluten- and non-gluten-related diseases, such as irritable bowel syndrome (IBS). We retrospectively selected patients with DL linked to IBS. Formalin-embedded biopsy samples of the duodenum were collected. CD3 lymphocyte immunohistochemistry was used for IELs. The real-time polymerase chain reaction was used to quantify the amount of mRNA coding for tissue transglutaminase 2 (tTG2), interferon-gamma (IFNγ), toll-like receptor 2 (TLR2), and myeloid differentiation primary response 88 (MyD88). All subjects underwent DQ2-8 haplotype analysis. Controls were represented by subjects with IBS without DL. Thirty-two patients with IBS-DL were retrospectively recruited. Fourteen subjects (43.8%) had a DQ2-8 haplotype. DQ2-8 positive subjects had similar levels compared to negative ones for tTG2, IFNγ, TLR2, and MyD88. Cigarette smoke did not influence molecular expression in our study. Smokers had a statistically higher IELs count than non-smokers (54.2 ± 7.7 vs. 36.0 ± 8.8, < 0.001). A significant, direct correlation between IELs and duodenal expression of IFNγ was found (r = 0.36, = 0.04). IBS with DL showed higher expression of inflammatory markers than controls, but DQ2-8 haplotype did not seem to affect their expression. Smoking might increase IELs infiltration.

The rise in food intolerances and celiac disease, along with advanced diagnostic techniques, has prompted health professionals to seek effective and economical testing methods. This study evaluates combining genetic tests with routine carbohydrate-absorption breath tests to classify patients with chronic gastrointestinal disorders into therapeutic groups, enhancing dietary management and improving gut health and quality of life. Forty-nine patients with suspected carbohydrate intolerance underwent genetic testing for lactase non-persistence, hereditary fructose intolerance, and celiac disease risk.

Is There A Connection Between Primary Hypophysitis and Celiac Disease?

Exp Clin Endocrinol Diabetes

January 2024

Istanbul University-Cerrahpasa, Cerrahpasa Medical Faculty, Department of Endocrinology, Metabolism, and Diabetes, Istanbul, Turkey.

Article Synopsis
  • The aim of the study was to explore the connection between primary hypophysitis (PH) and celiac disease (CD) by analyzing patient records and clinical findings from 2007 to 2022.
  • The study included 19 patients with lymphocytic hypophysitis, 30 celiac patients, and 30 healthy controls, finding no cases of CD in patients with PH and lower HLA-DQ2/8 detection rates in PH compared to CD.
  • Results indicated significant differences in antibody levels between the groups, suggesting a potential link between autoimmune responses in PH and genetic factors associated with celiac disease.

Article Synopsis
  • The study explores the genetic factors influencing the risk and protection against type 1 diabetes (T1D), focusing on specific HLA genotypes DQ2/8 and DQ6/8.
  • It proposes that DQ6 plays a protective role by "stealing" islet epitopes, limiting the ability of the disease-susceptible DQ8 to present those peptides.
  • The findings suggest a unique peptide-binding mechanism in DQ6 that competes with DQ8, providing insight into how protective genotypes can influence immune responses to prevent T1D.

Objectives: Assessing the seroprevalence and the prevalence of definite coeliac disease (CD) in the German LIFE Child Health study cohort including immunoglobulin A (IgA) antibodies against tissue transglutaminase (IgA-TTG) in addition to IgG antibodies against deamidated gliadin peptides (IgG-DGP) and human leukocyte antigen (HLA)-DQ2/8 genotyping.

Methods: Samples from children and adolescents were first screened for IgA-TTG and IgG-DGP. If IgA-TTG was above 0.
