Congenital hernia of the diaphragmatic dome (CDC) is a congenital embryopathy where all or part of the diaphragmatic dome fails to form. Its rate is 1/3500 live births with a male predominance. This study reports a case of Bochdalek hernia as well as data in the literature and highlights the diagnostic and therapeutic challenges in this disease. Diagnosis and antenatal management are essential to reduce patient's mortality and morbidity. Prognosis is poor and depends on the presence and degree of pulmonary hypoplasia and on the association with congenital malformations.

Lung ultrasound has become indispensable in managing critically ill patients, offering bedside evaluation capabilities for intensive care unit physicians without ionizing radiation. This noninvasive technique demonstrates high sensitivity and specificity in diagnosing various lung pathologies, including pleural effusion and consolidation syndrome. A novel trans-hepatic subcostal approach enhances visualization of the lower right lung lobe, revealing the diaphragm dome and inferior lobe through oblique coronal sections.

Emphysematous liver abscesses usually present with fever and abdominal pain with radiological investigations showing air inside the abscess cavity and biochemical parameters suggestive of sepsis. This is a condition that needs urgent intervention, but it can present with variable presentations and the gas under the right dome of the diaphragm makes the diagnosis in dilemma, confusing it with hollow viscous perforation. Hereby we present a case series of variable presentations of emphysematous liver abscesses, successfully managed by timely intervention.

Congenital hernia of the diaphragmatic dome is defined as a congenital embryopathy where all or part of the diaphragmatic dome fails to form during embryonic development. The classification is based on the location of the defect in the diaphragm. In this report, we present a case of Morgani hernia in a 52-year-old man whose sole initial symptom was dyspnea, occurring within the context of a SARS-CoV-2 (COVID-19) infection.
