Introduction: Fasciculations are most commonly seen in the biceps brachii muscle in amyotrophic lateral sclerosis (ALS). In this study we have explored the association between fasciculation frequency in a single location-biceps brachii and brachialis muscles (BB), and disease burden and activity.

Methods: Sonographic muscle studies were performed in 90 ALS patients, 47 of whom were seen in subsequent follow-up. The association between fasciculations frequency at the BB and ALS Functional Rating Scale-Revised (ALSFRS-R) and manual muscle testing (MMT) scores was determined.

Results: High fasciculation frequency at the BB, where detection rate was the highest, was associated with shorter disease duration, greater muscle thickness, higher MMT scores, and faster rate of decline in ALSFRS-R initially, and MMT subsequently.

Discussion: High fasciculation frequency at the BB as determined by sonography, is associated with less impairment at time of examination, and a more active disease with a more rapid progression.

Ultrasonography for Assessment and Intervention With Botulinum Toxin Injection for Tremors.

Ann Rehabil Med

December 2024

Department of Rehabilitation Medicine, National Rehabilitation Center, Ministry of Health and Welfare, Seoul, Korea.

Objective: Tremors are caused by contractions of reciprocally innervated muscles. The role of ultrasound in diagnosing tremors has not yet been investigated, although it appears to be promising because it can visualize muscle movements. In the present study, we report four cases of tremor (Holmes' tremor, extremity tremor associated with palatal myoclonus, dystonic tremor, and tremor associated with dystonia), which were evaluated using ultrasound and treated with botulinum toxin injections.

Muscle ultrasound aids diagnosis in amyotrophic lateral sclerosis.

Clin Neurophysiol

November 2024

Brain and Nerve Research Centre, Concord Clinical School, University of Sydney, Concord Hospital, Sydney, NSW, Australia.

Objective: There is a need for improved diagnostic tools in Amyotrophic Lateral Sclerosis (ALS). Our objective was to assess muscle ultrasound as a diagnostic tool in patients with ALS and determine a simplified screening protocol to aid implementation in clinical practice.

Methods: Ultrasound of bulbar and limb muscles was prospectively performed on all patients referred to a single centre with suspected ALS.

Article Synopsis
  • - The study explored how the presence of fasciculation potentials (FPs) in electromyography (EMG) and uric acid (UA) levels relate to survival in amyotrophic lateral sclerosis (ALS) patients, revealing a link between these markers and disease progression.
  • - Researchers classified 89 ALS patients into three groups based on their progression speed: fast (less than 1 year), average (1 to 3 years), and slow (3 years or more), finding significant differences in FPs and UA levels across these groups.
  • - Higher percentages of affected muscles with FPs and lower UA levels were strongly correlated with faster disease progression and shorter survival, emphasizing the need for better prognostic indicators in ALS.

Botulinum toxin as an effective treatment for persistent twitching in first toe: a detailed case study.

J Int Med Res

September 2024

Department of Anaesthesiology and Pain Medicine, The Catholic University of Korea, St Vincent's Hospital, Suwon, Korea.

Continuous fasciculation that occurs without weakness is referred to as benign fasciculation. Although generally considered non-threatening, cases that persist can significantly impact an individual's quality of life. This study presents a case of a male patient in his 30s experiencing unyielding twitching localized to the sole of his left foot for 2 years.

Dr Muhammad Ashraf Hosen, Junior Consultant (Medicine), 100 Beded Upazila Health Complex, Modhupur Tangail, Bangladesh; E-mail:

Organophosphorus compound (OPC) poisoning is common in Bangladesh. The toxicity of the agent and paucity of appropriate medical services has resulted in high mortality rates. To find out the clinical profile and outcome of OPC poisoning patients is the main aim of my study.
