We treated a 60 - year - old man with pes planovalgus due to rheumatoid arthritis. He had been suffering from left foot pain with swelling. Despite drug therapy, his foot pain and deformity had got worsen. Taking into consideration his skin and bone quality, arthroscopic triple arthrodesis was performed. To access the subtalar joint, 2 portals were applied at the sinus tarsi, and decortication was performed. For calcaneocuboid joint, 1.5 cm portal was applied along with joint line at calcaneocuboid joint. Calcaneocuboid joint was fully decorticated, then, 1.5 cm portal was applied at the joint line of talonavicular joint in parallel. Synovectomy and decortication under arthroscopy were performed. Once each joint was sufficiently prepared, it was fixed using screws a percutaneous stab incision with an autologous bone graft from the iliac crest to the calcaneocuboid and talonavicular joint. At 12 weeks postoperatively, bone union was confirmed. The Japan Society for Surgery of the Foot (JSSF) RA foot and ankle scale had improved from the postoperative value of 38 points to a postoperative score of 86 points at one year. Plain radiographs showed that good alignment of the patient's hindfoot was maintained. We found that arthroscopic approach was able to achieve satisfactory outcome and minimise soft tissue trauma in a compromised patient.

We present a case of a unique midfoot injury pattern including dislocations of the calcaneocuboid and naviculocuneiform joints with associated calcaneus and navicular fractures after low-energy injury. This combination of injuries is rare, especially with a low-energy mechanism. There are no current treatment guidelines.

This contribution details a new high-fidelity finite element analysis (FEA) methodology for the investigation of the effect of the graft size on the pressure distribution developing at the calcaneocuboid joint after the Evans osteotomy procedure. The FEA model includes all 28 bones of the foot up to the distal end of fibula and tibia as well as soft tissues, tendons, and muscles. The developed FEA model was validated by comparing the in-vivo pressure distribution on the foot plantar with the in-silico results, resulting in a low deviation equal to 7.

Foot tuberculosis is rarely reported in the literature, with most tuberculosis of the foot being an uncommon manifestation of skeletal tuberculosis. Early diagnosis and timely medical and surgical intervention can significantly reduce morbidity. A 23-year-old male presented with persistent swelling and pain in his right foot for six months, accompanied by a discharging sinus over the affected area in the last week, making weight-bearing increasingly difficult.
