Purpose: Telepharmacy (TF) represents a tool to manage the complexity related to drug therapies and to guarantee continuity of patient empowerment in transitions of care. The aim of this work is to analyze the critical issues related to the implementation of the TF service, in order to enhance its application.
Results: Over 27 months, the TF service was offered to 60 patients.

Digital healthcare has rapidly evolved during and in the post-COVID pandemic era, expanding the roles and responsibilities of community pharmacists. Services like telepharmacy, e-prescriptions, remote medication therapy management, and digital monitoring of chronic conditions, have evolved into everyday routine pharmacy practices. Pharmacists are at the forefront and the most accessible healthcare professionals for patients and are increasingly pivotal in providing comprehensive patient care, including digital patient care services.

Background: Community pharmacists are now the most accessible healthcare professionals, providing advice, information, drugs, and devices across the globe during the COVID-19 pandemic. In Thailand, accredited community pharmacies meet higher standards than qualified community pharmacies, but little is known about the perspectives of accredited community pharmacists and patients in this emergency situation. This study aimed to assess pharmacists' and patients' perspectives on the challenges and opportunities they faced in providing or receiving patient care and services during the COVID-19 pandemic.
