We present a case of a 55-year-old man with a history of poorly differentiated metastatic thyroid cancer, which is rare and shows aggressive behavior. The patient had radioactive iodine-negative multiple metastasis on 18F-FDG PET/CT, therefore 68Ga-DOTATATE and 68Ga-PSMA PET/CT were performed whether an opportunity exist for DOTATATE or PSMA-directed radionuclide therapy. Different patterns of tumoral uptakes were detected on 18F-FDG, 68Ga-DOTATATE, and 68Ga-PSMA PET/CT, as a demonstration of tumor heterogeneity.

Objectives: The objective of this study was to assess receptor expression in metastatic differentiated thyroid carcinoma patients with progressive elevated thyroglobulin and negative iodine scintigraphy, we used 68 Ga-DOTATATE [Gallium-68 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid (DOTA)-octreotate (DOTATATE)] (Krenning's score) and 68 Ga-PSMA-11 (Gallium-68 prostate-specific membrane antigen-11) PET-computed tomography (CT) [molecular imaging prostate-specific membrane antigen (miPSMA) score]. Patients with Krenning's score 3 and above and miPSMA score 2 and above were considered to determine the incidence of patients, who would qualify for treatment with 177 Lu-DOTATATE/PSMA [Lutetium-177 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid (DOTA)-octreotate (DOTATATE)/prostate-specific membrane antigen]-based therapy. In addition, we compared 68 Ga-DOTATATE and 68 Ga-PSMA-11 PET-CT with 2-deoxy-2-[F-18]fluoroglucose ( 18 F-FDG) PET-CT (using maximum standardized uptake value).

Methods: Thirty-nine patients with pathologically confirmed NPC were enrolled in this prospective study. Each patient underwent paired [Ga]Ga-DOTATATE and [Ga]Ga-FAPI PET/CT on 2 successive days.

Background: Gallium-68 is a positron emitter for PET applications that can be produced without cyclotron by a germanium (Ge-68) chloride/gallium (Ga-68) chloride generator. Short half-life (67.71 min) of Ga-68, matching pharmacokinetic properties of small biomolecules, facilitates isotope utilization in compounding radiopharmaceuticals for PET imaging.

68Ga-PSMA and 68Ga-DOTATATE PET/CT have shown promising performance in diagnosing prostate cancer and neuroendocrine tumors, but there are also pitfalls. We report a case of a 78-year-old man with prostate lesions showing intense uptake of 18FDG, 68Ga-PSMA, and 68Ga-DOTATATE simultaneously, with heterogeneous enhancement on contrast-enhanced CT and abnormal signal changes on PET/MRI. It was finally diagnosed as granulomatous prostatitis.

The molecular PET-CT imaging profile of an interesting case of differentiated thyroid carcinoma, later transformed into with thyroglobulin elevation and negative iodine scintigraphy (TENIS) with tyrosine kinase inhibitor (TKI) resistant recurrent aggressive disease, is presented. The patient was evaluated to assess SSTR-2 or PSMA expression to explore the possibility of any effective targeted nuclear therapy. F-FDG, Ga-DOTATATE and Ga-PSMA-11 PET/CT was performed, which revealed tracer avidity in all 3 scans in the extensive loco-regional disease of large ill-defined retropharyngeal and retro-tracheal soft tissue eroding cricoid cartilage, extending into tracheal lumen and left sided strap muscles.
