Background: Ultracongruent (UC) total knee replacement (TKR) designs, serving as alternatives to posterior stabilized (PS) and cruciate retaining (CR) designs, lack conclusive evidence regarding posterior femoral rollback. A 33-year-old male patient developed distal femur chronic osteomyelitis with massive bone loss after an open grade-3b fracture. Following several failed treatments to eradicate infection, a tibial turn-up procedure was performed to provide a stable and functional stump. To avoid neurovascular problems, the popliteal vessels and sciatic nerve were moved medially, and the flap was rotated externally to decrease the collapse.

Cutting tools with orderly arranged diamond grits using additive manufacturing show better sharpness and longer service life than traditional diamond tools. A retractable needle jig with vacuum negative pressure was used to absorb and place grits in an orderly arranged manner. However, needle hole wear after a long service time could not promise complete grit adsorption forever.
