Background: Therefore, the objectives of this study were to investigate the prevalence of carbapenem-resistant hypervirulent (CR-hvKp) in Fujian Medical University Union Hospital, identify their genetic characters, characterize their resistance profiles, and identify risk factors for their infection to improve prevention and treatment strategies for CR-hvKp in the area.
Methods: Between January 2021 and January 2022, clinically identified carbapenem-resistant (CRKp) isolates were collected. A PCR assay was used to detect the K capsule type, virulence genes, carbapenemase genes, and membrane pore protein.

Rationale: Carbapenem-resistant Klebsiella pneumoniae (CRKP) bloodstream infections are a severe complication resulting from granulocyte deficiency following chemotherapy for hematologic malignancies and have a high mortality rate. However, reports of disseminated organ infections secondary to bloodstream infections are rare.
Patient Concerns And Diagnoses: We report 2 cases of patients with acute lymphoblastic leukemia who both developed CRKP bloodstream infections during the granulocyte deficiency stage following chemotherapy, with 1 case of secondary bacterial liver abscess and 1 case of secondary septic arthritis.

Carbapenemase OXA-48 and its variants pose a serious threat to the development of effective treatments for bacterial infections. OXA-48-producing Enterobacterales are the most prevalent carbapenemase-producing bacteria in large parts of the world. Although these bacteria exhibit low-level carbapenem resistance , the infections they cause are challenging to treat with conventional therapies, owing to their spread and complex detection in clinical settings.
