Purpose: Patients with single ventricle anatomy palliated with Fontan operation are at risk for thromboembolism, arrhythmia, and heart failure rendering pregnancy high risk or even contraindicated. Infertility and high rates of first trimester miscarriage are not uncommon. In vitro fertilization (IVF) with or without gestational surrogacy can be an option, but poses risks during ovarian stimulation, oocyte retrieval, and the post-procedural period. We present six cases of women with complex congenital heart disease status post Fontan operation who underwent successful IVF.
Methods: Case series from a single-center tertiary care setting.
Results: Indications for referral were cardiac or fertility concerns for pregnancy of the congenital cardiologist. One woman had mild volume overload after oocyte retrieval requiring furosemide and one experienced post-operative colitis. There were no thrombotic complications.
Conclusions: A multidisciplinary team-based approach can result in successful oocyte retrieval and IVF in women with complex congenital heart disease and Fontan physiology.

This study investigated whether luteinizing hormone receptor (LHR) expression varies in the granulosa cells of individual follicles according to the maturation stage of the oocytes harvested for assisted reproductive technology (ART) treatment. We observed minimal to no LHR mRNA and protein expression in cumulus cells surrounding oocytes arrested in the germinal vesicle (GV) stage. Interestingly, their ability to mature was confirmed by rescue in vitro maturation, suggesting somatic cell LHR deficiency as a key factor for the retrieval of GV oocytes in ART procedures.

Research Question: To what extent do legislative measures impact standard reproductive outcome parameters?
Design: Retrospective cohort study using data from the Swiss national IVF registry analysing the outcomes of 13,908 women undergoing embryo transfers resulting from their first lifetime oocyte retrieval before (2014-2016) or after (2020-2022) revision of the legislation, allowing extended culture for 12 zygotes. Live birth rates (LBR) and cumulative LBR (cLBR) were compared in fresh and frozen embryo transfer strategies in both periods. Adjusted multivariable mixed model analyses were performed to determine OR and incidence rate ratios (IRR).

Objective: To build a prediction nomogram for early prediction of live birth probabilities according to number of oocytes retrieved in women ≤ 35 years of age.
Methods: A prediction model was built including 9265 infertile women ≤ 35 years of age accepting their first ovum pick-up cycle from January 2018 to December 2022. Least absolute shrinkage and selection operator (LASSO) regression was performed to identify independent predictors and establish a nomogram to predict reproductive outcomes.
