17-Estradiol (E2) is a natural steroidal estrogen essential for a variety of physiological functions in organisms. However, external E2, which is renowned for its potent biological effects, is also considered to be an endocrine-disrupting compound (EDC) capable of disturbing the normal operation of the endocrine system, even at nanogram-per-liter (ng/L) concentrations. Studies have revealed that medical and livestock wastewater can be contaminated with E2, which poses potential risks to human health.

Acute heart failure is associated with high hospitalization and mortality rates. A strong, independent risk factor for mortality in patients with heart failure is acute kidney injury, and the condition caused by this connection between disturbances in heart function and proper kidney functioning is cardiorenal syndrome (CRS). Hypercalcemia is a common electrolyte disturbance, most frequently caused by hyperparathyroidism or malignancy, though it can also arise from adrenal insufficiency, creating diagnostic challenges. We present a case of a 78-year-old male patient with stage 3b chronic kidney disease due to immunoglobulin A nephropathy who exhibited altered mental status following dehydration caused by a five-day episode of diarrhea. The patient presented with hypercalcemia and acute kidney injury.
