Mohs Micrographic Surgery Efficacy for Solid Organ Transplant Head and Neck Cutaneous Squamous Cell Carcinoma.

Otolaryngol Head Neck Surg

January 2025

Department of Otolaryngology-Head and Neck Surgery, Lewis Katz School of Medicine, Philadelphia, Pennsylvania, USA.

Objective: Solid organ transplant (SOT) recipients carry a higher incidence of cutaneous squamous cell carcinoma (cSCC) with more aggressive features and worse outcomes compared to immunocompetent (IC) patients. The National Comprehensive Cancer Network advocates peripheral and deep en-face margin assessment such as Mohs micrographic surgery (MMS) for very-high-risk cSCC. We aim to assess the efficacy of MMS in the treatment of SOT immunosuppressed head and neck (HN) cSCC patients.

Introduction: Atopic dermatitis (AD) is a common and relapsing skin disease characterized by skin barrier dysfunction, inflammation, and chronic pruritus. Both cutaneous barrier dysfunction and immune dysregulation are critical etiologies of the pathology of AD. Although various anti-inflammatory pharmacological agents, including cytokine inhibitors and signaling pathway blockers, have been developed recently, keeping the skin clean is of utmost importance in maintaining physiological cutaneous barrier function and avoiding an AD flare.

Pyoderma gangrenosum (PG) is a rare inflammatory neutrophilic dermatosis that is difficult to diagnose due to non-specific clinical, laboratory, and histopathologic features. Distinguishing pseudoepitheliomatous hyperplasia (PEH) from squamous cell carcinoma (SCC) is also histopathologically challenging. The connection between PEH and PG is not well recognized, and instances of PG mimicking SCC are rare.

Disparities in Skin Cancer Outcomes in the Latine/Hispanic Population.

Despite having an overall lower lifetime risk for skin cancer, Latine/Hispanic individuals experience increased morbidity and mortality in skin cancer outcomes compared to non-Hispanic White individuals. The reasons for these disparate outcomes are multifactorial, but challenges in early skin cancer detection, limited awareness of risks, and inequitable access to care and/or treatment among this patient population likely are contributory. In this article, we review cutaneous malignancies in the Latine/Hispanic population and explore factors that impact overall prognosis, including unique clinical features, inadequate health coverage, medical mistrust, language barriers, differing cultural perspectives, and inadequate research.
