The developmental origins of disease or fetal programming model predict that early (intrauterine and/or postnatal) exposures to external insults of sufficient length and intensity may have enduring or lifelong consequences for physical and psychological health. The method described in this chapter considers an animal model to study the pathophysiological alterations connected to an HPA axis (hypothalamic-pituitary-adrenal) hyperactivity that are induced by an early-life stressful procedure involving the opioid system.

The number of people suffering from Substance Use Disorder (SUD) worldwide has increased 45 % compared to the last decade according to the latest United Nations World Drug Report. This staggering increase, partly due to the recent COVID-19 pandemic, further raises the social and economic burden for nations. Prevention and treatment, two of the main strategies employed to curb the increase in SUD, have shown limited success despite our increasing understanding of the underlying processes of SUD.

The Province of British Columbia (BC) is in the midst of an ongoing public health emergency, declared in 2016 in response to significant levels of drug poisonings/overdoses stemming from the unregulated drug supply. In response, BC implemented decriminalization in 2023, removing criminal sanctions for adults possessing up to 2.5 cumulative grams of opioids, cocaine, methamphetamine, and MDMA.

Introduction: Chronic pain is a leading cause of chronic disease in Australia, with a 2020 report indicating that one in five Australians aged over 45 experience chronic pain. The high prevalence of chronic pain accounts for significant healthcare utilization and associated costs, with the economic impact of chronic pain estimated to be AUD$139 billion in 2018.
Case Presentations: This paper uses two exemplar cases to demonstrate inadequacies within the current systems supporting those with chronic pain and the associated impacts these inadequacies have on patient outcomes and healthcare costs.
