Subungual or periungual cutaneous warts are caused by the human papillomavirus (HPV). These lesions can be refractory to destructive therapy, necessitating the use of intralesional immunotherapies such as antigen. In this case report, we present a 23-year-old female who sustained distal fingertip soft tissue necrosis following intralesional injection of antigen for the treatment of a refractory subungual wart.

The traditional mainstay method consists of topical steroids, which, if used excessively, may give rise to other concerns. Periungual wart is a kind of verruca vulgaris that occurs alongside or underlying the nails, which is challenging to treat and prone to recurrence, seriously affecting the quality of patients' life. We report a 6-year-old boy with periungual warts who had experienced various treatments with no improvement and even worsening. Therefore, we tried to treat the patient with local hyperthermia which uses a patented device that has an infrared emission source.

Successful treatment of periungual warts presents a clinical challenge, as many are refractory or represent following conventional treatments. The use of destructive therapies, such as cryotherapy, may even cause permanent nail dystrophy. Here, we present a series of nine cases in which intralesional cidofovir was used for recalcitrant periungual warts between July 2020 and July 2022 at the University of California, Los Angeles.

A 65-year-old woman developed erythema, papules and nodules over the body. Some nodules of her auricles and hands like string beads. Besides, she suffered from symmetrical swelling and pain of multiple joints, morning stiffness with deformity of joints; She had elevated erythrocyte sedimentation rate and C reactive protein levels; Her rheumatoid factor and antinuclear antibody were positive; Joints destruction was found with X-ray imaging; Skin pathology showed Dermal infiltrate of abundant histiocytes, part of them with a ground-glass appearance; A CD68 immunohistochemical stain was positive and the cells were negative for S100, CD1a.
