Published: September 2020

Objective: Leiomyosarcoma and ovarian cancer are often diagnosed late due to the absence of initial symptoms. Patients seek help when abdominal distension occurs; this is associated with pelvic tumor and carcinomatosis. Initial imaging often reveals pelvic tumors with diffuse abdominal nodules; however, this imaging could be misleading, such as in the cases of splenosis.

Case Report: A female presented with vaginal bleeding at our outpatient department. Serum CA125 level was elevated. Abdominal and pelvic CT showed multiple uterine masses and left adnexal cysts with peritoneal nodules. Leiomyosarcoma or ovarian cancer with carcinomatosis was suspected. Exploratory laparotomy was performed. Multiple purple spots spreading over peritoneal cavity were noted during the surgery. Pathological examination revealed adenomyosis with multiple uterine myomas and left ovarian endometrioma. Splenic tissues peritoneal implants were observed.

Conclusion: In patients with a history of spleen rupture or splenectomy, splenosis should be considered in the differential diagnosis, especially in young patients.

Department of Pathology, Saveetha Medical College and Hospitals, Saveetha Institute of Medical and Technnical Sciences, Saveetha University, Chennai, IND.

Primary ovarian leiomyosarcoma (LMS) is an extremely rare and aggressive tumor that originates in the smooth muscle tissue of the ovary. Typically affecting older women, it is diagnosed at an advanced stage and is associated with a poor prognosis, with most patients succumbing within the first year. The effectiveness of adjuvant chemotherapy and radiotherapy remains unclear due to the rarity of reported cases.

Article Synopsis
  • The study evaluated a new MRI diagnostic algorithm designed to distinguish between benign uterine leiomyomas and malignant uterine sarcomas in atypical cases.
  • Researchers reviewed 40 cases of atypical uterine masses and found that the algorithm had strong accuracy metrics, with sensitivity and specificity rates around 75% and 95%, respectively.
  • The results indicate that the consensus MRI algorithm is effective, achieving even higher accuracy when focusing on leiomyosarcomas, suggesting it could be a valuable tool for radiologists.

On the Origin of Abdominal Venous Leiomyosarcomas: The Role of the Sex-Hormone Drainage Pathways.

World J Oncol

October 2024

Department of Medical Imaging, University of Toronto, Princess Margaret Cancer Center, University Health Network, Toronto, ON, Canada.

Article Synopsis
  • The study investigates abdominal venous leiomyosarcoma (AV-LMS), focusing on its origins from veins associated with the sex-hormone drainage pathway (SHDP).
  • A retrospective analysis of imaging from 155 patients with confirmed non-uterine abdominal/pelvic LMS revealed that more than half of the tumors originated from venous structures, particularly those linked to the SHDP, including adrenal and ovarian veins.
  • Key findings indicate that tumors from the SHDP are prevalent and may lead to a lower risk of distant metastases at diagnosis, with tumor size and SHDP origin being critical factors in predicting metastasis risk.
