Objectives: In this retrospective study, we investigated whether first trimester serum placental growth factor (PIGF) differed amongst pregnancies with placenta previa-accreta and non-adherent placenta previa and healthy pregnancies.

Methods: In 1 January 2017-30 September 2019, a total of 177 pregnant females were included in the study, as follows: 35 cases of placenta previa-accreta, 30 cases of non-adherent placenta previa, and 112 cases of age and BMI-matched, healthy pregnant controls. PIGF multiples of the median (MoM) were acquired from laboratory data files. The predictor of placenta accreta was analyzed by using multiple logistic regression analysis.

Results: PIGF MoM of placenta previa-accreta group was significantly higher than those of the non-adherent placenta previa group and control group (p = 0.0098 < 0.01, p = 0.0002 < 0.01). Serum PIGF was found to be significantly positively associated with placenta accreta after adjusted gestational week at time of blood sampling, BMI, and age (OR: 4.83; 95% CI: 1.91-12.24;p = 0.0009 < 0.01). In addition, previous cesarean section history (OR: 2.75; 95% CI: 1.23-6.17; p = 0.014 < 0.05) and smoking (OR: 9.17; 95% CI: 1.69-49.62; p = 0.010 < 0.05) were also significantly associated with placenta accreta.

Conclusion: Increased first trimester serum PIGF was significantly positively associated with placenta accreta, suggesting that the potential role of PIGF in identifying pregnancies at high risk for placenta accreta. Previous cesarean section history and smoking may be the risk factors for accreta in placenta previa patients.

Uterine artery embolization in the management of postpartum hemorrhage.

World J Emerg Surg

January 2025

The Research Office, College of Medicine and Health Sciences, United Arab Emirates University, Al Ain, United Arab Emirates.

Background: Postpartum hemorrhage (PPH) is one of the leading preventable causes of maternal morbidity and mortality causing one-fourth of all maternal deaths. We aimed to study the role of uterine artery embolization (UAE) in controlling PPH and its impact on the need for hysterectomy.

Methods: We studied patients who were diagnosed with primary PPH between February 2012 and March 2020 at Al Ain Hospital, United Arab Emirates.

Fluid resuscitation strategy in patients with placenta previa accreta: a retrospective study.

Front Med (Lausanne)

September 2024

Department of Obstetrics and Gynecology, West China Second University Hospital, Sichuan University, Chengdu, Sichuan, China.

Article Synopsis
  • Obstetric hemorrhage is a leading cause of maternal death, with placenta previa accreta being a significant factor, contributing to 66% of such cases requiring fluid resuscitation.
  • The study retrospectively examined risk factors, clinical features, and outcomes by analyzing hemodynamic indicators related to patients diagnosed with placenta previa accreta over key time points during medical procedures.
  • Key findings indicate that conditions like placenta increta/percreta and delivering after 37 weeks significantly increase the risk of massive hemorrhage in these patients, highlighting the need for effective monitoring and fluid management strategies.

Cesarean hysterectomy in pregnancies complicated with placenta previa accreta: a retrospective hospital-based study.

BMC Pregnancy Childbirth

October 2024

Department of Obstetrics and Gynecology, West China Second University Hospital, Sichuan University, Chengdu, China.

Article Synopsis
  • Placenta previa accreta (PPA) can lead to severe blood loss during cesarean hysterectomy, sometimes requiring the procedure to prevent life-threatening hemorrhage.
  • A study of 199 women undergoing cesarean sections found that factors like emergency surgery, cervical invasion of the placenta, and bladder injury increased the risk of excessive blood loss.
  • Conversely, techniques like balloon occlusion of the iliac arteries and aortic balloon occlusion appeared to reduce the risk of significant blood loss during surgery.

Objectives: Placenta previa-accreta spectrum disorders are a cause of obstetric hemorrhage that can lead to maternal fetal mortality and morbidity. We aimed to describe the use of a uterine isthmic tourniquet left as a new uterus-preserving approach for patients with placenta previa-accreta.

Methods: In this retrospective comparative study, the patients who underwent surgery for placenta previa between 2017 and 2024 at our tertiary hospital were reviewed.

In the case of placenta previa-accreta when the placenta covers the entire anterior uterine wall, it is difficult to avoid transecting the placenta by traditional low-transverse cesarean section (CS), resulting in catastrophic hemorrhage and fetal anemia. To prevent this critical risk, we developed the CS with transverse uterine fundal incision (TUFI) and this technique has been widely used as a beneficial surgical method in clinical practice owing to its safety advantages for the mother and neonate since our first report. However, the risk of uterine rupture during a subsequent pregnancy remains unclear.
