Background: As a result of the constant increase in carbapenem resistance amongst gram-negative bacteria in several countries, the inappropriate use of carbapenems must be reduced. Antimicrobial stewardship programmes (ASPs) aim to improve carbapenem usage by implementing interventions, including the promotion of the de-escalation (DE) strategy. Thus, this study aimed to evaluate the impact of this strategy on carbapenem use based on a clear definition of DE.

Methods: The post-prescription review and feedback (PPRF) strategy, which is used to optimise carbapenem use, was implemented by the antimicrobial stewardship team (AST). We compared the DE rate during the pre-AST intervention period (from April 2017 to March 2018) and post-AST intervention period (from April 2018 to March 2019).

Result: A total of 1500 patients (n = 771 in the pre-AST intervention period and n = 729 in the intervention post-AST period) were admitted to the hospital. The average duration of antibiotic therapy decreased from 9.9 to 7.7 days. The DE rate significantly increased in the post-AST intervention period compared with the pre-AST intervention period (51.4% vs 40.3%; P < .001).

Conclusion: The PPRF strategy implemented by the AST could improve the carbapenem usage by increasing the DE rate of carbapenem.

Background: In developed countries, most of the neurologists use pericranial nerve blocks to treat headache patients, nevertheless, the knowledge and use patterns of this technique in developing countries are unknown.

Objective: Evaluate the knowledge and use patterns of pericranial nerve blocks in headache treatment by Mexican neurologists.

Material And Methods: We did a cross-sectional study, 90 Mexican neurologists completed a 26-question survey including data about sociodemographics, knowledge and patterns of use of pericranial nerve blocks.

Distinct genetic risk loci between biopsy-proven renal and non-renal lupus: a 10-year longitudinal cohort.

Rheumatology (Oxford)

January 2025

Division of Rheumatology & Clinical Immunology, Department of Medicine, Queen Mary Hospital, The University of Hong Kong, Hong Kong.

Objective: Systemic lupus erythematosus (SLE) is a heterogeneous disease which manifests as different subphenotypes. Distinct subphenotypes, such as lupus nephritis (LN), have been associated with increased genetic risk, but prior studies have been limited by cross-sectional and imprecisely subphenotyped cohorts. This study investigated the genetic basis for LN using a longitudinal cohort of distinctly subphenotyped patients.

The clinical assessment of lung involvement in patients with Still's disease, results from the multicentre international AIDA Network Still's Disease Registry.

Rheumatology (Oxford)

Department of Medical Sciences, Surgery and Neurosciences, Research Center of Systemic Autoinflammatory Diseases and Behçet's Disease Clinic, University of Siena, Siena, Italy.

Objectives: To assess the lung involvement in patients with Still's disease, an inflammatory disease assessing both children and adults. To exploit possible associated factors for parenchymal lung involvement in these patients.

Methods: A multicentre observational study was arranged assessing consecutive patients with Still's disease characterized by the lung involvement among those included in the AIDA (AutoInflammatory Disease Alliance) Network Still's Disease Registry.

Seroprevalence of peste des petits ruminants in sheep and goats managed under pastoral and agro-pastoral systems.

J Infect Dev Ctries

December 2024

SACIDS Africa Centre of Excellence for Infectious Diseases, SACIDS Foundation for One Health, Sokoine University of Agriculture (SUA), P.O. Box 3297 Chuo Kikuu, Morogoro, Tanzania.

Introduction: Peste des petits ruminants (PPR) is an infectious disease that imposes substantial economic burdens on small ruminants (SR) production. For Tanzania to develop efficient management and eradication plans, it is essential to comprehend the seroprevalence of PPR designated for global elimination by 2030.

Methodology: This study investigated the prevalence of PPR in animals kept under pastoral and agropastoral communities in Tanzania.

Whole-grain foods (WGFs) constitute a large part of humans' daily diet, making risk identification of WGFs important for health and safety. However, existing research on WGFs has paid more attention to revealing the effects of a single hazardous substance or various hazardous substances on food safety, neglecting the mutual influence between individual hazardous substances and between hazardous substances and basic information. Therefore, this paper proposes a causal inference of WGFs' risk based on a generative adversarial network (GAN) and Bayesian network (BN) to explore the mutual influence between hazardous substances and basic information.
