AI Article Synopsis

  • Inversion of the Left Atrial Appendage (LAA) is a rare condition linked to negative pressure inside the heart.
  • High speeds from Left Ventricular Assist Devices (LVADs) can create this negative pressure, increasing the risk of LAA inversion.
  • LAA inversion can result in serious complications, including blockage of the mitral valve and failure of the right ventricle, especially after LVAD surgery.

Article Abstract

• Inversion of the LAA is very rare and is associated with negative intracardiac pressure. • High LVAD speeds induce negative intracardiac pressure and can cause LAA inversion. • LAA inversion can lead to mitral valve outflow obstruction and RV failure. • This has the potential to be a catastrophic complication after LVAD implantation.

Background: Daptomycin pharmacokinetics and pharmacodynamics data relative to higher doses in patients are necessary for clinical practice.

Objectives: A monocentric, prospective study that enrolled patients with a diagnosis of spp. infective endocarditis treated with daptomycin according to clinical practice, to evaluate the pharmacokinetics/pharmacodynamics of different daptomycin daily doses (group A: 8-10 and group B: 11-12 mg/kg).

A 9-y-old, spayed female, non-brachycephalic mixed-breed dog was presented with progressive abdominal distension and diarrhea of <24 h duration. An echocardiogram revealed nodular masses in the pericardium and myocardium, particularly near the auricles. Diagnostic and therapeutic pericardiocentesis failed to improve the dog's status, and the owner elected euthanasia.

B-type natriuretic peptide (BNP) levels accurately reflect the degree of cardiac overload in heart failure. Considering cardiac morphology and intracardiac pressure, including the left ventricular end-systolic volume index (LVESVI) and left ventricular end-diastolic volume index (LVEDVI), is essential for cardiac overload assessment. These indexes influence plasma BNP levels, and high heart rate is likely associated with cardiac morphology.

Polyangiitis overlap syndrome (POS) is a systemic vasculitis characterized by overlapping features of more than one well-defined vasculitic syndrome. We present the case of a 38-year-old Malay man with progressive dyspnea and palpable purpura in his lower limbs. The diagnostic evaluation revealed right-sided segmental pulmonary consolidation with pleural effusion, systolic cardiac dysfunction with the presence of an intracardiac thrombus, and left vocal cord palsy secondary to laryngeal mononeuropathy.

Totally implantable venous access devices (TIVADs) are frequently used in people with cystic fibrosis as a means of securing consistent vascular access, particularly in the context of severe disease and microbial colonization. Infection of TIVADs is not uncommon and typically associated with coagulase negative staphylococci, though infection with other organisms does occur too. We report on the first case of a TIVAD infection caused by in person with cystic fibrosis.
