Spontaneous coronary artery dissection (SCAD) is a non-traumatic non-iatrogenic coronary dissection. It's a frequent cause of acute coronary syndrome (ACS) in women without or with few traditional cardiovascular risk factors. We report an unusual case of multivessel SCAD in a middle-aged woman with successful medical management.

The guide extension-facilitated ostial stenting (GEST) technique uses a guide extension catheter (GEC) to improve stent delivery during primary coronary angioplasty (PCI). GECs are used for stent delivery into the coronary arteries of patients with difficult anatomy due to tortuosity, calcification, or chronic total occlusion (CTO) vessels. Stent and balloon placement has become challenging in patients with increasing lesion complexity due to tortuosity, vessel morphology, length of the lesion, and respiratory movements.

Genetics of intracerebral hemorrhage.

J Cereb Blood Flow Metab

McCance Center for Brain Health, Massachusetts General Hospital, Boston, MA, USA.

Spontaneous intracerebral hemorrhage(ICH) represents a life-threatening form of stroke, marked by its impact on survival and quality of life. ICH can be categorized from monogenic disorders linked to causal germline variants in ICH-related genes to complex sporadic cases, highlighting the interaction among lifestyle factors, environmental influences, and genetic components in determining risk. Among sporadic ICH, the influence of these factors varies across ICH subtypes, evidenced by heritability rates of up to 73% for lobar ICH versus 34% for non-lobar ICH.

Hypermagnesemia- and Hyperphosphatemia-Associated Cardiac Arrest after Injection of a Novel Magnesium-Based Bone Cement in Spinal Surgery.

January 2025

From the Department of Orthopaedic Surgery, Singapore General Hospital, Singapore (Dr. Loh, Dr. Ling, Dr. Jiang, and Lim) and the Department of Surgical Intensive Care, Division of Anaesthesiology and Perioperative Medicine, Singapore General Hospital, Singapore (Dr. Goh).

We report a case of pulseless electrical activity (PEA) associated with profound hypermagnesemia immediately after cementation of a novel magnesium-based cement in spine surgery. During T8 to T12 posterior instrumentation and decompression laminectomy for vertebral metastasis secondary to lung cancer, a 61-year-old Chinese woman developed sudden hypotension and went into PEA immediately after injection of a novel magnesium-based cement. Intraoperative fluoroscopic imaging did not show any notable cement extravasation.

Article Synopsis
  • ECPR has enabled previously ineligible patients with refractory out-of-hospital cardiac arrest (OHCA) from acute myocardial infarction (AMI) to undergo primary percutaneous coronary intervention (PCI).
  • A study examined 671 patients over five years, finding that 37% of them had refractory cardiac arrest despite ECPR, with about 65% achieving return of spontaneous circulation (ROSC) post-PCI and 21% surviving to discharge.
  • Key factors influencing survival included having prehospital ROSC, quicker ECPR initiation, and achieving a high TIMI flow grade after the procedure.
