Importance: Surgical quality improvement efforts have largely focused on 30-day outcomes, such as readmissions and complications. Surgery may have a sustained impact on the health and quality of life of patients considered frail, yet data are lacking on the long-term health care utilization of patients with frailty following surgery.

Objective: To examine the independent association of preoperative frailty on long-term health care utilization (up to 24 months) following surgery.

Advancements in the Management of Fragility Fractures in Orthopaedic Patients.

Cureus

November 2024

Orthopaedics, Glasgow Royal Infirmary, Glasgow, GBR.

Osteoporosis is a major risk factor for fragility fractures. The British Orthopaedics Association Standards for Trauma and Orthopaedics (BOAST) and Getting it Right First Time (GIRFT) guidelines on fragility fracture management highlight the need to initiate prompt, coordinated multidisciplinary care with a focus on early mobilisation to improve patient outcomes. Medical management of fragility fractures focuses on the prevention of progressive frailty.

Primary malignant melanoma of the esophagus (PMME) is a rare malignancy typically associated with poor prognosis, particularly in elderly patients. Here, we present the case of an 85-year-old female patient with a three-month history of progressive dysphagia and heartburn-related epigastric pain. Endoscopy revealed a polypoid esophageal lesion, confirmed as melanoma via biopsy with positive immunohistochemical staining for Melan-A and SOX-10.

Approximately 320 million surgeries occur annually worldwide, increasingly performed on an ageing, comorbid population in whom postoperative complications contribute significantly to mortality. While anesthesiologists have led advances in perioperative care, the optimal structure of the provision of postoperative care has lacked discourse. In this article, we describe the implementation, structure, role, and benefits of an Anesthesiology Perioperative Outreach Service (APOS) at a Canadian tertiary hospital, providing proactive daily review and management of high-risk surgical patients.

Evaluating frailty scores across experimental groups in rodent models: bridging physical and cognitive domains.

FEBS Open Bio

December 2024

Department for Neurobiology, Institute for Biological Research 'Sinisa Stankovic', National Institute of Republic of Serbia, University of Belgrade, Belgrade, Serbia.

Frailty, a reversible clinical geriatric syndrome, impairs the ability to maintain homeostasis, leading to severe consequences such as hospitalization and death. Cognitive frailty, characterized by the co-occurrence of physical frailty and cognitive impairment, has garnered increasing attention in recent years. Preclinical models, especially rodent studies, are essential for understanding frailty and developing interventions to mitigate associated conditions.
