Objective: As per the eighth edition of the American Joint Committee on Cancer (AJCC) staging system for differentiated thyroid carcinoma (DTC), minimal extrathyroidal extension (mETE) has been removed. Instead, gross ETE (gETE) invading only strap muscles has been designated as a new T3b category. Our objective was to investigate the impact of the T3b category on survival in order to establish its prognostic value in DTC.

Design: In this retrospective study, we included patients who had undergone thyroidectomy between 2004 and 2012. Data from the Surveillance, Epidemiology and End Results (SEER) database were examined.

Methods: We used the Kaplan-Meier method and log-rank test to analyse overall survival (OS) and cancer-specific survival (CSS). The effect of potential predictors associated with survival were estimated using the Cox regression model. To minimize selection bias, propensity-score matching (PSM) was performed.

Results: A total of 63 315 patients were included in our study. During the average follow-up duration of nearly 78 months, significant differences were observed in cancer-specific survival among patients with no ETE, mETE, gETE invading only strap muscles (T3b) and gETE invading perithyroidal structures other than strap muscles (T4) (P < .05). In univariable and multivariate analysis, both mETE and T3b exhibited significant poorer CSS compared with no ETE. After adjusting for patient features with PSM, it was confirmed that T3b was associated with worse CSS compared with no ETE and mETE.

Conclusions: Both mETE and gETE are independent factors for DTC, implying that the new T3b category is worthy of reference for medical workers. Furthermore, mETE was significantly associated with poorer outcome. Our conclusion may provide support for the modification of the TNM staging system in the future.

  • The study investigates the management of IVC tumor thrombus in renal cell carcinoma patients and aims to establish a minimum annual caseload for better surgical outcomes.
  • Researchers analyzed data from 2006-2020, revealing that hospitals performing at least 8 procedures annually had significantly lower in-hospital mortality rates (2.8%) compared to those with fewer than 8 cases (6.2%).
  • Factors influencing mortality included patient age, ventilation time, and hospital caseload, highlighting the importance of experience and volume in surgical success.*

Background: Ampulla of Vater carcinoma (AVC) stage T3 was subdivided according to the degree of pancreatic invasion into T3a (≤ 0.5 cm) and T3b (> 0.5 cm) by the 8th edition of the Union for International Cancer Control (UICC)/American Joint Committee on Cancer (AJCC) cancer staging system.

Article Synopsis
  • The study investigates the significance of tumor size in T3b differentiated thyroid cancer (DTC) by analyzing data from 6,282 patients who had thyroid surgery between 2000 and 2017.
  • T3b tumors were categorized into three groups based on size—T3b-1 (≤2 cm), T3b-2 (2-4 cm), and T3b-3 (>4 cm)—to evaluate their impact on survival outcomes.
  • Findings suggest that T3b-1 tumors show survival rates similar to T1 tumors, indicating a potential need to reclassify T3b-1 as T1 for better prognostic accuracy in cancer staging.

Sentinel lymph node biopsy (SLNB) is a surgical procedure aimed to detect nodal metastases in patients with clinically occult disease. Since the advent of new systemic therapies, its role in melanoma has been extensively debated over the last years. In this article, three possible scenarios are discussed, considering the SLNB impact on the management of melanoma patients.

Purpose: Pathologic T3b (pT3b) prostate cancer, characterized by seminal vesicle invasion (SVI), exhibits variable oncological outcomes post-radical prostatectomy (RP). Identifying prognostic factors is crucial for patient-specific management. This study investigates the impact of bilateral SVI on prognosis in pT3b prostate cancer.
