The meaning of borderline rejection in kidney transplantation.

Borderline T cell-mediated rejection is a diagnostic category of the Banff schema that uses a lower diagnostic threshold. Subclinical borderline T cell-mediated rejection detected by surveillance sampling is not entirely benign. Studies demonstrate increased rates of late rejection, progressive nephron loss, functional impairment, donor-specific antibody formation, and allograft failure. It can be conceptualized as a failure to control the cellular alloimmune response. Mixed results are reported for its treatment, and randomized controlled trials are needed.

The outcome of thrombotic microangiopathy in kidney transplant recipients.

BMC Nephrol

November 2024

Department of Medicine, Johns Hopkins Hospital, The Johns Hopkins University School of Medicine, 600 Wolfe St. Carnegie 344B, Baltimore, MD, 21287, USA.

Background: The outcome of kidney transplant recipients with a history of complement-mediated thrombotic microangiopathy (cTMA) and those who develop post-transplant de novo TMA (dnTMA) is largely unknown.

Methods: We retrospectively studied all kidney transplant recipients with end-stage kidney disease secondary to cTMA and those who developed dnTMA, between Jan 2000 and Dec 2020 in our center.

Results: We identified 134 patients, 22 with cTMA and 112 had dnTMA.

(1) Background: donor-derived circulating free DNA (dd-cfDNA), an innovative biomarker with great potential for the early identification and prevention of graft damage. (2) Methods: Samples were collected prospectively and the study was performed retrospectively to analyze dd-cfDNA plasma levels in 30 kidney transplant patients during their post-transplant follow-up (15 days, 3, 6, and 9 months), to determine if the result could be of interest in the identification of possible adverse events, especially rejection. The aim was to verify whether the data on sensitivity, specificity, NPV, and PPV compare with reference values and creatinine values.

Background: The spectrum of histological findings in transplanted kidneys from HIV-positive donors to HIV-positive recipients is relatively unexplored. This study describes the type and timing of histological diagnoses observed in this unique cohort.

Methods: Adequate biopsies were analyzed at implantation and posttransplant between September 2008 and May 2022.

Article Synopsis
  • ABO-incompatible kidney transplantation (KT) is increasingly used to address organ shortages, but high levels of anti-ABO antibodies can hinder successful transplantation despite existing desensitization methods.
  • Eculizumab was used as an additional treatment to help patients with very high anti-ABO antibodies who did not respond to standard therapies, showing improvements in kidney function and antibody levels after transplant.
  • The study indicates that short-term eculizumab treatment may effectively support ABOi KT patients by maintaining stable kidney function and reducing antibody levels, even in cases previously deemed challenging.

Background: In kidney transplantation, molecular diagnostics may be a valuable approach to improve the precision of the diagnosis. Using next-generation sequencing (NGS), we aimed to identify clinically relevant archetypes.

Methods: We conducted an Illumina bulk RNA sequencing on 770 kidney biopsies (540 kidney recipients) collected between 2006 and 2021 from 11 European centers.
