Background: Desmoplakin (DSP) pathogenic/likely pathogenic (P/LP) variants are associated with malignant phenotypes of arrhythmogenic cardiomyopathy (DSP-ACM). Reports of outcomes after ventricular tachycardia (VT) ablation in DSP-ACM are scarce.

Objectives: In this study, the authors sought to report on long-term outcomes of VT ablation in DSP-ACM.

Purpose: The left atrial posterior wall (LAPW) can be a target for atrial fibrillation (AF) catheter ablation but is sometimes difficult to completely isolate due to the presence of endocardial-epicardial connections. We aimed to investigate the incidence and distribution of epicardial residual connections (epi-RCs) and the electrogram characteristics at epi-RC sites during an initial LAPW isolation.

Methods: We retrospectively studied 102 AF patients who underwent LAPW mapping before and after a first-pass linear ablation along the superior and inferior LAPW (pre-ablation and post-ablation maps) using an ultra-high-resolution mapping system (Rhythmia, Boston Scientific).

Unlabelled: In patients with a history of antithrombin deficiency, a direct thrombin inhibitor may be considered as an alternative to intraprocedural anticoagulation. A 55-year-old woman with antithrombin deficiency and prior myocardial infarction suffered from electrical storm and appropriate defibrillator shocks. Substrate mapping guided left ventricular endocardial and epicardial ablations were successfully performed.

Background: Inferoseptal process of the left ventricle (ISP-LV) might be a source of idiopathic ventricular arrhythmias. In these cases, ectopic foci are accessible from the LV endocardium, epicardially from the middle cardiac vein as well as from the right atrium (RA). This study reports a series of patients with premature ventricular contractions (PVCs) arising from the ISP-LV that were successfully ablated following access from different structures.

Article Synopsis
  • This study looked at how effectively doctors can treat ventricular arrhythmias located deep in the heart's tissue using a procedure called ablation.
  • Out of 92 patients with intramural outflow tract PVCs, 75% achieved immediate suppression of the arrhythmias, and the overall burden of PVCs significantly decreased after the procedure.
  • The research shows that while it’s challenging to ablate these arrhythmias, many patients benefit from specialized techniques, and about a third may need repeat procedures within the following months.
