The medial-basal segment (S7) is the smallest of all the pulmonary segments and exhibits significant anatomic complexities. The procedures adopted to perform anatomic S7 segmentectomy differ according to branching pattern as the B7 may show 3 types. Although some studies have reported surgical procedures for thoracoscopic segmentectomy of the S7 and its anatomic variants, this type of segmentectomy remains challenging.

Objectives: The anatomic features and surgical techniques focusing on the right medial-basal segment (RS7) are few reported. This study aimed to accurately define the new nomenclature and classifications of B7 anatomy, elucidate its prevalence and anatomical characteristics and summarize the surgical outcomes.
Methods: Between August 2019 and February 2022, 5023 patients were admitted for pulmonary nodules.

Herein, we report the first case of a patient with lung cancer with an aberrant medial basal segmental pulmonary artery (A7b) behind the superior segmental pulmonary vein (V6) who underwent right superior segment (S6) segmentectomy via uniportal video-assisted thoracoscopic surgery (uVATS). A 56-year-old man with a right lower lobe pure ground-glass nodule (GGN), measuring 12 mm in diameter on computed tomography (CT) had an aberrant A7b branching from the basal pulmonary artery, which was located behind the V6 as detected on 3D CT. The right S6 segmentectomy, via uVATS, for the GGN was performed.
