Signals of a Quantum Universe.

Structure in the Universe is widely believed to have originated from quantum fluctuations during an early epoch of accelerated expansion. Yet, the patterns we observe today do not distinguish between quantum or classical primordial fluctuations; current cosmological data are consistent with either possibility. We argue here that a detection of primordial non-Gaussianity can resolve the present situation, and provide a litmus test for the quantum origin of cosmic structure. Unlike in quantum mechanics, vacuum fluctuations cannot arise in classical theories and therefore long-range classical correlations must result from (real) particles in the initial state. Similarly to flat-space scattering processes, we show how basic principles require these particles to manifest themselves as poles in the n-point functions, in the so-called folded configurations. Following this observation, and assuming fluctuations are (i) correlated over large scales and (ii) generated by local evolution during an inflationary phase, we demonstrate that the absence of a pole in the folded limit of non-Gaussian correlators uniquely identifies the quantum vacuum as the initial state. In the same spirit as Bell's inequalities, we discuss how this can be circumvented if locality is abandoned.

Migrasomes, the vesicle-like membrane micro-structures, arise on the retraction fibers (RFs), the branched nano-tubules pulled out of cell plasma membranes during cell migration and shaped by membrane tension. Migrasomes form in two steps: a local RF bulging is followed by a protein-dependent stabilization of the emerging spherical bulge. Here we addressed theoretically and experimentally the previously unexplored mechanism of bulging of membrane tubular systems.

Clinical Pharmacokinetics of Atogepant in Healthy Japanese and White Adults.

Introduction: Atogepant is a calcitonin gene-related peptide receptor antagonist approved for the preventive treatment of migraine in adults in the USA, EU, and several other countries. The objectives of this study were to evaluate the pharmacokinetics (PK) and dose proportionality of atogepant in healthy Japanese participants, evaluate the safety and tolerability of atogepant in Japanese participants, and explore the differences in the PK and safety of atogepant in Japanese vs white participants.

Methods: A total of 50 participants (40 Japanese and 10 white) were enrolled into five cohorts; Japanese cohorts were randomized in a 4:1 ratio to atogepant (10 mg, 30 mg, or 60 mg daily dosing and 60 mg twice daily) or placebo.

High flow nasal cannula (HFNC) can reduce the need for intubation in patients with coronavirus disease-19 (COVID-19) pneumonia induced acute hypoxemic respiratory failure (AHRF), but predictors of HFNC success could be characterized better. C-reactive protein (CRP) and D-dimer are associated with COVID-19 severity and progression. However, no one has evaluated the use of serial CRP and D-dimer ratios to predict HFNC success.

Background: Management of the adult airway is one of the most stressful and time-critical procedures in emergency medicine. In the Cowichan District Hospital, a rural hospital in British Columbia, Emergency Department (ED) staff were uncomfortable with acquiring the equipment needed for adult advanced airway management and the mean length of time to acquire the equipment was 319 s. The aim of this quality improvement (QI) project was to decrease the time to obtain the equipment needed for adult advanced airway management by nurses and physicians in the Cowichan District Hospital ED to less than 90 s by May 2023.
