Pneumonia is a severe health problem causing millions of deaths every year. The aim of this study was to develop an advanced deep learning-based architecture to detect pneumonia using chest X-ray images. We utilized a convolutional neural network (CNN) based on VGG16 architecture consisting of 16 fully connected convolutional layers. A total of 5856 high-resolution frontal view chest X-ray images were used for training, validating, and testing the model. The model achieved an accuracy of 96.6%, sensitivity of 98.1%, specificity of 92.4%, precision of 97.2%, and a F1 Score of 97.6%. This indicates that the model has an excellent performance in classifying pneumonia cases and normal cases. We believe, the proposed model will reduce physician workload, expand the performance of pneumonia screening programs, and improve healthcare service.

An 80-year-old man with aquaporin-4-antibody-positive neuromyelitis optica spectrum disorder presented with a 2-week history of cough and hiccups, followed by progressive bilateral lower limb weakness, a bandlike burning sensation in the upper body, and urinary retention. Magnetic resonance imaging showed area postrema and thoracic central medullary lesions. Thorax computed tomography showed bilateral upper lung lobe consolidations.

In elderly patients, the onset of pneumonia is often subtle and typical respiratory symptoms are less common. Instead, non-specific presentations such as altered mental status and loss of appetite should be closely monitored. The sensitivity of chest X-rays and routine laboratory tests is often low, so a comprehensive assessment including risk factors is required.

Streptococcus pneumoniae (SP) remains an important cause of community acquired pneumonia (CAP). We aimed to describe the prevalence and characteristics of outpatients with radiologically confirmed pneumococcal CAP. Between November 2017 and December 2019, a French network of general practitioners enrolled CAP-suspected adults, with ≥1 clinical signs of infection and ≥1 signs of pulmonary localization in an observational study.

Sjogren's syndrome is an autoimmune condition characterized by infiltration of exocrine glands but, in rare cases, can have extraglandular involvement with pleural effusion being an exceedingly rare form. Here we present a case of Sjogren's pleuritis resulting in pleural effusion, a rare initial presentation for Sjogren's syndrome. A woman in her 20s presented to the emergency department after a recent hospitalization for pneumonia, pleural effusion, and pulmonary embolism.
