A 29-year-old Ukrainian woman presented to the obstetric clinic at 28 weeks' gestation with pregnancy complicated by intrauterine growth restriction. She reported progressively worsening dyspnea during her pregnancy and was found to have significant hypoxia with an oxygen saturation of 84% on room air prompting admission for further evaluation. Oxygen saturation improved to 92% on 10 L of supplemental oxygen. On further questioning, she was found to have a history significant for pleurodesis as a treatment for recurrent pneumothoraces and nephrectomy for a benign renal mass several years prior while living in Ukraine.

Background: Chronic-kidney-disease (CKD) is prevalent among adults undergoing noncardiac surgery, with surgery-related factors potentially worsening CKD or triggering acute kidney injury (AKI). We hypothesized that CKD patients experience more kidney function decline within one to two years post-surgery than those without CKD, particularly if they develop AKI.
Methods: We conducted a single-center retrospective cohort study, including noncardiac surgery patients with documented creatinine preoperative and between 1 and 2 years after surgery.

Shoulder abscesses, commonly resulting from bacterial infections, can occasionally present with atypical etiologies and delayed onset. We report a rare case of a massive polymicrobial shoulder abscess developing two decades after an insect bite, emphasizing its clinical presentation, diagnostic approach, and surgical management. A 65-year-old female presented with severe, progressively worsening right shoulder pain, a 20 cm swelling, and purulent discharge persisting for 15 days.
