Anterior inferior tibiofibular ligament (AITFL) lesion have been shown to result in proliferation of cicatricial tissue ; concomitant insufficiency of the anterior talofibular ligament (ATFL) and AITFL Lesion may cause anterolateral syndesmotic impingement in the ankle joint of runners. Twenty-two runners with suspected syndesmotic impingement after ankle sprain were included in the study. An MRI of the ankle joint was performed followed by arthroscopy. Arthroscopy revealed an ATFL lesion in 20 patients (87%) and anterolateral syndesmotic impingement in 17 patients (77%). An ATFL lesion was detected in all patients with anterolateral syndesmotic impingement. The sensitivity of MRI was 24% (4 patients) on detecting anterolateral syndesmotic impingement, and 25% (5 patients) on ATFL lesion. A traumatic sprain of the ankle frequently results in a combined ATFL lesion and anterolateral syndesmotic impingement in runners. The abilities of MRI to detect this combined pathology are limited. Arthroscopy of the ankle joint should be performed. Study Design : Case series ; level of evidence 4.

Article Synopsis
  • Syndesmotic impingement syndrome is linked to ongoing lateral or anterolateral ankle pain, commonly occurring after a rotational injury.
  • A study analyzed 107 ankles of patients who underwent surgery for this syndrome, assessing the types of tissue involved and their relationship with cartilage damage.
  • The findings revealed that fibrotic tissue was the most common type of impingement and was significantly associated with cartilage lesions in the lateral talar dome, indicating the need for careful evaluation in patients with prolonged ankle pain.

Article Synopsis
  • This study aimed to investigate how common osteochondral lesions (OCLs) are in the lateral talar dome among patients with anterior ankle impingement who also have a thickened segment of the anterior tibio-fibular ligament.
  • Researchers reviewed the cases of 40 patients who underwent arthroscopy for ankle impingement, assessing various clinical outcomes before and after surgery.
  • The results showed that 90.6% of patients had a hypertrophic ligament, and 75.9% had OCLs, with postoperative scores improving significantly, indicating a link between the ligament thickness and the presence of OCLs.

Background: Posttraumatic osteonecrosis (ON) of the lateral distal tibia is a rare but severe complication of malleolar fractures. Treatment options include ankle fusion, arthroplasty, osteotomy, and drilling but clinical data on outcomes are limited to single case reports. The aim of this study was to prospectively evaluate the outcome following joint-preserving reconstruction.

Current Trends in Total Ankle Replacement.

Ankle arthritis can result in significant pain and restriction in range of motion. Total ankle replacement (TAR) is a motion-preserving surgical option used as an alternative to total ankle arthrodesis to treat end-stage ankle arthritis. There are several generations of TAR techniques based on component design, implant material, and surgical technique.

A wide spectrum of pathologies can lead to soft tissue abnormalities within the ankle joint. Many of these disorders can develop into irreversible joint degeneration if left untreated. Arthroscopy is frequently used to treat these soft tissue conditions such as instability, synovitis, impingement, arthrofibrosis, and other inflammatory disorders in the rearfoot and ankle.
