Purpose Of Review: Fluid management in hemorrhagic shock is a controversial topic, and there are evolving clinical guidelines and evidence-based practice. This review aims to highlight the physiological aspects in the light of current evidence on which volume replacement solution to use.
Recent Findings: Current evidence and international guidelines are shifting from a liberal to a restrictive fluid resuscitation strategy, emphasizing the potential risks associated with aggressive fluid therapy.

Lactic acidosis is an uncommon metabolic complication of malignancy, often associated with high tumor burden and increased mortality, and more frequently observed in hematologic malignancies than in solid tumors. This case report describes a patient with newly diagnosed signet ring cell carcinoma of the cecum, an uncommon and aggressive histological subtype of colon cancer, complicated by severe type B lactic acidosis. A 66-year-old female patient with primary signet ring cell carcinoma of the cecum and peritoneal carcinomatosis underwent a right colectomy with extended small bowel resection.

Prenatal severe Ebstein anomaly might be complicated by a circular shunt. In these cases, persistently elevated right atrial and venous pressure (due to severe tricuspid regurgitation) is complicated by a systemic ineffective blood shunt via a DA, resulting in diminished end-organ perfusion and acidosis, due to overall low cardiac output. Affected fetuses are at a significantly higher risk of intrauterine fetal demise.

Introduction: Slush ice drinks are commonly available refreshments, aimed at children and young people. Glycerol is used to maintain the slush effect in the absence of a high sugar content.
Objective: To describe a series of children who became acutely unwell shortly after consuming a slush ice drink; their presentation mimics specific inherited metabolic diseases (IMDs).

Ethylene glycol, a common component in automotive antifreeze and various household and industrial products, poses significant health risks upon ingestion, whether accidental or intentional. Characterized by severe metabolic acidosis, calcium oxalate crystal formation, and diverse end-organ damage, ethylene glycol toxicity can be fatal, with a potentially lethal dose estimated at 1500 mg/kg. The parent compound is osmotically active, leading to the production of harmful metabolites, such as glycolic and oxalic acids, which contribute to metabolic acidosis, nephrotoxicity, and cardiac toxicity.
