Introduction: Roux-en-Y gastric bypass is the most common bariatric procedure in Belgium. Retrograde intussusception is identified as a rare late complication.
Methods: Here we present two cases of retrograde jejuno-jejunal intussusception after Roux-en-Y gastric bypass.
Results: Women who experienced excellent weight loss appear most prone. Although ectopic pacemaking is suggested, the exact pathophysiologic mechanism remains unclear. Simple manual reduction seems to be insufficient as treatment. Lowest recurrence rates are noted after segmentectomy.
Conclusion: Retrograde intussusception is a rare late complication after RYGB which is becoming more and more relevant due to increasing procedure volumes. Retrograde peristalsis by ectopic pacemakers could be identified as a cause. Segmentectomy appears to be the best treatment of choice.

Background: Cholesterol efflux capacity (CEC) of HDL (high-density lipoprotein) is inversely associated with incident cardiovascular events, independent of HDL cholesterol. Obesity is characterized by low HDL cholesterol and impaired HDL function, such as CEC. Bariatric surgery, including Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG), broadly leads to improved cardiovascular outcomes, but impacts on risk factors differ by procedure, with greater improvements in weight loss, blood pressure, and glycemic control after RYGB, but greater improvements in HDL cholesterol and CEC levels after SG.

The management of gastrointestinal anastomotic leaks post surgery is a considerable challenge, characterized by elevated morbidity and mortality, particularly in cases of esophageal-jejunal anastomotic leaks. Diverse endoscopic intervention techniques have been utilized with enhanced success. We present a case where a 57-year-old patient with Siewert type II esophageal cardia cancer underwent endoscopic deployment of a fully covered stent into a fistula resulting from anastomotic leakage, following a laparoscopic proximal gastrectomy with Roux-en-Y and double tract reconstruction.
