Objectives: Unilateral diaphragmatic paralysis (UDP) due to phrenic nerve injury is a potential complication of thoracic surgery. This study evaluated the prevalence of UDP associated with surgical mitral valve repair (MVR) and its effect on surgical outcomes in dogs.

Animals, Materials And Methods: Two hundred ninety-four dogs that underwent MVR were included in the study. A retrospective review of medical records was performed for dogs surviving surgery. Diagnosis of UDP was based on preoperative and postoperative thoracic dorsoventral radiographs.

Results: A total of 284 dogs survived until the day after surgery. The prevalence of UDP on the day after surgery, on the day of discharge, and after the first postoperative month was 30%, 24%, and 9%, respectively. One case of UDP was observed at 3 months after surgery. Unilateral diaphragmatic paralysis was exhibited by nine of the 21 patients that died in the hospital. The proportion of patients with UDP was higher in dogs that died of respiratory failure than in dogs that died of other causes (p = 0.002). Most dogs whose deaths were suspected to have been related to respiratory failure also had pre-existing respiratory diseases. The occurrence of UDP did not relate to the lengths of stay in the intensive care unit or the hospital.

Conclusions: Our findings suggest that UDP is a common complication in dogs after MVR and that the prevalence of UDP decreases with time after surgery. Unilateral diaphragmatic paralysis is a risk factor for postoperative death, especially in patients with pre-existing respiratory disease.

Ovarian surface epithelial cancer (OSEC) are an entity in which, according to genomics and pathology data accumulated in the last couple of decades, several different nosological entities with distinct etiologies are aggregated. In ovarian cancer, surgery is the pivot of treatment, to which medical oncological treatment is added by recommendation in most cases. This is a single centre sample of 263 cases with OSEC operated from January 2014 until December 2021 with a 28-month period of follow-up, until 30th April 2024.

Unlabelled: Diaphragm dysfunction develops in central nervous system damage, chest injuries, complications of cardiac surgery, long-term artificial lung ventilation, respiratory diseases. Anatomical morphological features of phrenic nerves allow to effectively use electromagnetic stimulation methods for functional recovery of the diaphragm in different pathological conditions. Invasive and non-invasive, electric and magnetic methods of stimulation are used depending on the severity of manifestations of the diaphragm dysfunction and its genesis.

Article Synopsis
  • Ultrasound-guided interscalene (IS) block provides effective pain relief for shoulder surgeries but can lead to hemidiaphragmatic paralysis and breathing difficulties.
  • This study compared the efficacy of a superior trunk (ST) block with the traditional IS block in patients undergoing humerus surgery, finding significantly lower rates of diaphragmatic paresis with the ST block.
  • Both blocks offered similar pain relief and opioid usage post-surgery, but the ST block is recommended as a safer alternative for patients with breathing issues.

Article Synopsis
  • Unilateral diaphragmatic paralysis (UDP) often occurs after cardiac surgery, particularly on the left side, but this case highlights a rare instance of right-sided UDP following endoscopic coronary artery bypass grafting (CABG).
  • A 60-year-old male patient was observed with breathing difficulties after his CABG, revealing diminished sounds in the right lung and a diagnosis of UDP and obstructive sleep apnoea syndrome (OSAS).
  • The treatment plan included inspiratory muscle training and CPAP therapy to improve breathing and address the effects of OSAS, while also discussing the significance of ultrasound in diagnosing and monitoring diaphragmatic dysfunction post-surgery.

Article Synopsis
  • Interscalene brachial plexus block is a common anesthetic method used for surgeries on the shoulder, clavicle, and humerus, but it carries risks of complications.
  • A case report highlights a patient who experienced unilateral diaphragmatic paralysis after this anesthesia technique, which was quickly diagnosed and treated using ultrasound.
  • The report emphasizes the growing importance of ultrasound in monitoring anesthesia practices and managing complications from nerve blocks.
