Background: Effect and Safety of Electroconvulsive Therapy plus Usual Care for the Acute Management of Severe Agitation in Dementia (ECT-AD) is a multi-site NIA-funded FDA-regulated pioneering clinical trial to investigate the effectiveness of electroconvulsive therapy (ECT) in treating severe and treatment-refractory agitation and aggression among individuals with advanced dementia, a condition that has a profound negative impact on patient quality of life and caregiver burden. Here we present baseline demographics of the patient population in this ongoing trial.

Method: To date we have enrolled 18 participants, with a mean age of 74.

Cerebral beta-amyloid accumulation is the key initiator of Alzheimer's disease (AD) pathology. Most familial early-onset AD mutations in the APP, PSEN1/2 genes increase the ratio of Abeta42:Abeta40, which drives beta-amyloid accumulation in the brain. In 2001, the late Steve Wagner, Maria Kounnas, and I directed an agnostic high-throughput screen for compounds that would reverse the Abeta42:Abeta40, ratio, and discovered the first non-NSAID (second generation) gamma secretase modulators (GSM) at TorreyPines Therapeutics.

Background: BXCL501, a sublingual film formulation of dexmedetomidine, a highly selective α2 adrenoceptor agonist, is currently being studied for the acute treatment of agitation associated with dementia.

Method: This was a Phase 1b/2 study assessing efficacy and tolerability of BXCL501 for treatment of acute agitation associated with dementia. Subjects were randomized to active treatment with BXCL501 (30, 40, or 60µg) or placebo.

Background: People living with dementia (PLWD) are high utilizers of acute illness and emergency care, with over 50% of the more than 6 million people with Alzheimer's disease and Alzheimer's disease related dementia (ADRD) visiting an emergency department (ED) annually. While the ED plays an important role meeting the urgent and acute needs of PLWD and their caregivers, presence of ADRD is often not well recognized and ED visits are associated with significant adverse outcomes for PLWD. Despite these factors, research on the emergency care needs of PLWD is extremely limited.
