Introduction: Postoperative urinary retention (POUR) is a common concern after total joint arthroplasty (TJA). However, overdiagnosis of POUR by bladder scans may lead to unnecessary interventions and associated complications. Anticholinergic toxicity typically presents with symptoms of cutaneous vasodilation, delirium, mydriasis, urinary retention, hyperthermia, anhidrosis, and tachycardia. This case report presents a 68-year-old female patient who exhibited some of these signs and symptoms after ingesting an unknown quantity of dicyclomine. However, she displayed one notable exception to the classic toxidrome.

Standard treatment includes placing a urinary catheter or a suprapubic catheter with outpatient urologic follow-up. Urine retention can cause complications, such as hyponatremia and post-obstructive diuresis.

This case report presents an 86-year-old female patient who developed a urinary tract injury and infection following a pelvic fracture caused by a bedside fall during hospitalization for  pneumonia. The patient experienced fever with chills and rigors, prompting antibiotic treatment. Imaging revealed an ischial tuberosity fracture with potential bone fragment retention in the bladder wall.
