Clinical and patient-reported outcomes in women offered oncoplastic breast-conserving surgery as an alternative to mastectomy: ANTHEM multicentre prospective cohort study.

Br J Surg

December 2024

Bristol Surgical and Perioperative Care Complex Intervention Collaboration, Translational Health Sciences, Bristol Medical School, University of Bristol, Learning and Research Building, Southmead Hospital, Bristol, UK.

Background: Oncoplastic breast-conserving surgery may be a better option than mastectomy, but high-quality comparative evidence is lacking. The aim of the ANTHEM study (ISRCTN18238549) was to explore clinical and patient-reported outcomes in a multicentre cohort of women offered oncoplastic breast-conserving surgery as an alternative to mastectomy with or without immediate breast reconstruction.

Methods: Women with invasive/pre-invasive breast cancer who were offered oncoplastic breast-conserving surgery with volume replacement or displacement techniques to avoid mastectomy were recruited prospectively.

Impact of Gender-confirming Chest Surgery on Sexual Health: A Prospective Study.

Plast Reconstr Surg Glob Open

August 2024

From the Department of Plastic and Hand Surgery, Centre Hospitalier Universitaire Vaudois, Lausanne, Switzerland.

Background: Few studies have evaluated the improvement of the sexual well-being of transgender patients after breast augmentation or bilateral mastectomy. This prospective study aims to assess the potential improvement in body awareness during sexual intercourse, as well as the nipple-areolar complex (NAC) sensitivity recovery after gender-confirming chest surgery.

Methods: From October 2019 up to April 2021, all transgender patients eligible for gender-confirming chest surgery in our institution were asked to fill out two questionnaires, namely BREAST-Q (a patient-reported outcome measure for use in reconstructive breast surgery and clinical practice) and Body Exposure during Sexual Activities Questionnaire (scientifically validated self-report assessments of body-image experiences in the context of sexual relations).

Background: Immediate breast reconstruction (IBR) with direct to implant (DTI) is the preferred method of reconstruction by many surgeons and patients, however, acellular dermal matrix (ADM) and other synthetic meshes are expensive especially in low- and middle-income countries. AIM OF THE WORK: To evaluate the technique, indications, aesthetic outcomes, and short and long-term complications of DTI breast reconstruction performed with Ultrapro®, a low-cost alternative mesh to ADM and other synthetic meshes.

Methods: Our study is a prospective cohort study that was conducted on 133 patients who experienced IBR following nipple-sparing mastectomy (NSM) or skin sparing mastectomy (SSM) using silicone implants and Ultrapro® mesh between December 2020 and December 2023.

Implants versus autologous tissue flaps for breast reconstruction following mastectomy.

Cochrane Database Syst Rev

October 2024

G.Re.T.A. Group for Reconstructive and Therapeutic Advancements, Milan, Italy.

Background: Women who have a mastectomy for breast cancer treatment or risk reduction may be offered different options for breast reconstruction, including use of implants or the woman's own tissue (autologous tissue flaps). The choice of technique depends on factors such as the woman's preferences, breast characteristics, preoperative imaging, comorbidities, smoking habits, prior chest or breast irradiation, and planned adjuvant therapies.

Objectives: To assess the effects of implants versus autologous tissue flaps for postmastectomy breast reconstruction on women's quality of life, satisfaction, and short- and long-term surgical complications.
