Aim: A few studies comparing laparoscopic and open techniques have reported that open repair with mesh is the optimal operation for unilateral primary hernia. The aim of this study is to compare the outcomes of laparoscopic transabdominal preperitoneal repair (TAPP) versus open mesh plug repair (MP) for bilateral primary inguinal hernia.

Methods: This was a retrospective study of 107 patients with bilateral primary inguinal hernia between January 2008 and December 2016. Of these patients, 49 underwent TAPP and 58 underwent MP. The surgical outcomes and the long-term outcomes using a questionnaire were compared between TAPP and MP.

Results: In the TAPP group, the operation time was significantly longer (103 vs 91 minutes;  = .019). The postoperative complication rate was not significantly different between the two groups. One patient (1.0%) in the TAPP group and five patients (4.3%) in the MP group suffered recurrence ( = .30). Postoperative groin pain was not significantly different (14% in the TAPP group vs 31% in the MP group;  = .065), but more patients required analgesics in the MP group (4.1% vs 17%;  = .036). The long-term outcomes, according to a questionnaire, were not significantly different between the two groups. The median follow-up period was 22 (range, 0.4-52) months in the TAPP group and 40 (range, 0.5-108) months in the MP group ( < .001).

Conclusion: TAPP for bilateral primary inguinal hernia achieved better results than MP relative to postoperative pain and the use of medication for pain relief without increasing the complication and recurrence rates.

Little influence of the abandoned sac on seroma formation following laparoscopic transabdominal preperitoneal repair of lateral inguinal hernia.

Background: The impact of completely reducing or transecting a hernia sac on seroma formation in laparoscopic surgery for lateral inguinal hernias remains debated. To date, no studies have compared the incidence of seroma in hernia sacs left untouched versus other surgical approaches. Abandoning the hernia sac involves avoiding manipulation of the inguinal canal, unlike the manipulation required for transection or reduction of the hernia sac.

Few metrics exist to describe phenotypic diversity within ophthalmic imaging datasets, with researchers often using ethnicity as a surrogate marker for biological variability. We derived a continuous, measured metric, the retinal pigment score (RPS), that quantifies the degree of pigmentation from a colour fundus photograph of the eye. RPS was validated using two large epidemiological studies with demographic and genetic data (UK Biobank and EPIC-Norfolk Study) and reproduced in a Tanzanian, an Australian, and a Chinese dataset.

The impact of COPD screening on US primary care clinician behavior and patient outcomes is unclear. Assess the impact of receiving CAPTURE (COPD Assessment in Primary Care to Identify Undiagnosed Respiratory Disease and Exacerbation Risk) screening scores on clinical and patient outcomes. Cluster randomized trial included 49 usual care (COPD education only) and 51 intervention (COPD and CAPTURE education plus screening scores) primary care practices.

Objective: Aim: To evaluate the early postoperative results of surgical treatment of patients with inguinal hernias after laparoscopic transabdominal preperitoneal inguinal hernia repair (TAPP) using invasive and non-invasive methods of fixation of prosthetic material.

Patients And Methods: Materials and Methods: The study included 76 patients (average age 61,4±15,3 years; 69 [91 %] males, 7 [9 %] females) with primary uncomplicated unilateral inguinal hernias who underwent TAPP. Patients were divided into two groups: group 1 - 52 patients in whom the AbsorbaTack fixation device was used to secure the mesh; group 2 - 24 patients in whom the LiquiBand FIX8 cyanoacrylate adhesive was used.

During the transition period, the incidence of diseases increases due to a negative energy balance that affects metabolic and immune status. Limiting milk production at the beginning of lactation by milking once a day or by incompletely milking twice a day improves the metabolic and immune status of cows. Previous studies have shown that milk production is highly responsive to additional MP.
