Purpose Of Review: This review summarizes recent literature for applying pharmacogenomics to antifungal selection and dosing, providing an approach to implementing antifungal pharmacogenomics in clinical practice.
Recent Findings: The Clinical Pharmacogenetics Implementation Consortium published guidelines on and voriconazole, with recommendations to use alternative antifungals or adjust voriconazole dose with close therapeutic drug monitoring (TDM). Recent studies demonstrate an association between phenotype and voriconazole levels, clinical outcomes, and adverse events. Additionally, -guided preemptive dose adjustment demonstrated benefit in two prospective studies for prophylaxis. Pharmacokinetic-pharmacodynamic modeling studies have generated proposed voriconazole treatment doses based on phenotypes, with further validation studies needed.
Summary: Sufficient evidence is available for implementing -guided voriconazole selection and dosing among select patients at risk for invasive fungal infections. The institution needs appropriate infrastructure for pharmacogenomic testing, integration of results in the clinical decision process, with TDM confirmation of goal trough achievement, to integrate antifungal pharmacogenomics into routine clinical care.

Purpose: To review recent advances with radiation therapy (RT) for soft tissue sarcomas (STS).
Recent Findings: Newer data showcases hypofractionated preoperative RT for soft tissue sarcomas treated with surgery to be safe and effective, however, long-term follow up data is pending. Hypofractionated and dose-escalated RT in patients with unresectable STS is also being studied, for which we remain optimistic given advances in RT planning approaches.

Introduction: The high percentage of Omicron breakthrough infection in vaccinees is an emerging problem, of which we have a limited understanding of the phenomenon.
Methods: We performed single-cell transcriptome coupled with T-cell/B-cell receptor (TCR/BCR) sequencing in 15 peripheral blood mononuclear cell (PBMC) samples from Omicron infection and naïve with booster vaccination.
Results: We found that after breakthrough infection, multiple cell clusters showed activation of the type I IFN pathway and widespread expression of Interferon-stimulated genes (ISGs); T and B lymphocytes exhibited antiviral and proinflammatory-related differentiation features with pseudo-time trajectories; and large TCR clonal expansions were concentrated in effector CD8 T cells, and clonal expansions of BCRs showed a preference for IGHV3.

Here, we report the rapid identification of human antibodies by high-throughput single-cell RNA and VDJ sequencing of memory B cells derived from 64 volunteers immunized with recombinant five-component vaccine (clinical phase I). From 676 antigen-binding IgG1 clonotypes, TOP10 sequences were selected for expression and characterization, with the most potent one, Abs-9, having nanomolar affinity for the pentameric form of the specific antigen protein A.
