The article describes an experience with the surgical treatment of repeated re-thromboses after aorto-femoral reconstructions with a bifurcation explant in 34 patients with the Leriche syndrome of atherosclerotic etiology who were subjected to 110 reoperations. Different kinds of reconstructions for revascularization of extremities are considered. Favorable results were obtained in 76.5% of the cases. Lethality was 2.73% per 110 operations. Long-term results were studied during 13 years. The bifurcation explant was found to be functioning in 30% of the cases observed.

Purpose: We sought to investigate the clinical results of preemptive temporary right ventricular assist device (RVAD) placement with left ventricular assist device (LVAD) implantation for anticipated right ventricular failure.

Methods: Patients at risk for right ventricular failure were identified by the heart team. After median sternotomy, Protekduo (LivaNova, London, UK) dual-stage cannula was inserted from the right internal jugular vein under fluoroscopy guidance, and advanced into the pulmonary artery to the bifurcation of the main pulmonary artery.

Mosquito Probing Enhances Dengue Virus Infection of Resident Myeloid Cells in Human Skin.

Department of Infectious Diseases and Microbiology, School of Public Health, University of Pittsburgh, Pittsburgh, PA 15261, USA.

The most prevalent arthropod-borne viruses, including the dengue viruses, are primarily transmitted by infected mosquitoes. However, the dynamics of dengue virus (DENV) infection and dissemination in human skin following probing remain poorly understood. We exposed human skin explants to adult female mosquitoes following their infection with DENV-2 by intrathoracic injection.

Temperature plays a crucial role in influencing the spatiotemporal dynamics of the heart. Electrical instabilities due to specific thermal conditions typically lead to early period-doubling bifurcations and beat-to-beat alternans. These pro-arrhythmic phenomena manifest in voltage and calcium traces, resulting in compromised contractile behaviors.

Circulating tumor cells (CTCs) and their clusters are the drivers of metastasis, but we have an incomplete understanding of how they interact with capillary beds. Using microfluidic models mimicking human capillary bifurcations, we observed cell size- and bifurcation-dependent shedding of nuclei-free fragments by patient CTCs, CTC-derived explant cells and numerous cancer cell lines. Shedding reduced cell sizes up to 61%, facilitating their transit through bifurcations.

The extravascular implantable cardioverter-defibrillator (EV ICD) with lead implantation in the substernal space may provide clinical advantages over transvenous and subcutaneous systems. This is the first reported examination of substernal infection in large animals implanted with the EV ICD system. The system was implanted in 13 large animals (canine, porcine, and ovine).
