Objective: To compare age and protein-energy malnutrition (PEM) - the predispositions - and fever and abnormal leukocyte count (ALC) - the SIRS criteria - in hospitalized children with and without diarrhea.
Design: A prospective case-control study.
Setting: A pediatric ward of a general hospital in a low-resource setting.
Participants: Totally, 445 consecutive admissions to the pediatric ward of a general hospital over a period of 1 year were included in this prospective case-control study; hemodynamically unstable subjects (11) were excluded.
Interventions: Age, PEM, fever, and ALC were assessed in 59 patients with diarrhea and compared with 375 control patients without diarrhea. Odds ratios with confidence intervals were determined; the chi-square test and binary logistic regression analysis were also performed.
Main Outcome Measures: Associations of diarrhea with age, PEM, fever and ALC singly and various combinations of predispositions and SIRS parameters.
Results: Infancy and ALC were significantly associated with diarrhea. PEM or fever alone was not significantly associated with diarrhea; however, the probability of developing diarrhea was significantly higher when a combination of ALC and PEM was observed. The combination of infancy, PEM, and ALC carried a sensitivity of 81·36%; for other combinations, sensitivity varied between 70% and 80%. The combination of infancy and ALC had the lowest sensitivity (59·32%) but the best specificity (61·07%).
Conclusion: The association/presence of a combination of SIRS parameters (fever and ALC) and predispositions (infancy and PEM) in children with diarrhea may help in deciding whether antibiotic therapy should be initiated.

Background: Studies on late community-acquired respiratory virus (CARV) infections in long-term allogeneic hematopoietic stem cell transplantation (allo-HCT) survivors are scarce, creating knowledge gaps on the epidemiology, risk of progression to lower respiratory tract disease (LRTD), and conditions linked to poor outcomes.
Patients And Methods: We included consecutive CARV infection episodes occurring up to six months after allo-HCT registered in our database from December 2013 to June 2023 at two Spanish transplant centers.
Results: Among 426 allo-HCT recipients, 1070 CARV episodes were recorded, 791 (74%) with only upper respiratory tract disease (URTD) and 279 (15%) progressing to LRTD, at a median of 18.

Background: Dental pain can have a detrimental effect on quality of life. Symptomatic apical periodontitis and acute apical abscess are common causes of dental pain and arise from an inflamed or necrotic dental pulp, or infection of the pulpless root canal system. Clinical guidelines recommend that the first-line treatment for these conditions should be removal of the source of inflammation or infection by local operative measures, and that systemic antibiotics are currently only recommended for situations where there is evidence of spreading infection (cellulitis, lymph node involvement, diffuse swelling) or systemic involvement (fever, malaise).

Objective: The clinical efficacy and safety of sorafenib in patients with advanced liver cancer (ALC) were evaluated based on transarterial chemoembolization (TACE).
Methods: 92 patients with ALC admitted to our hospital from May 2020 to August 2022 were randomly rolled into a control (Ctrl) group and an observation (Obs) group, with 46 patients in each. Patients in the Ctrl group received TACE treatment, while those in the Obs group received sorafenib molecular targeted therapy (SMTT) on the basis of the treatment strategy in the Ctrl group (400 mg/dose, twice daily, followed by a 4-week follow-up observation).
