Published: March 2020

This is a rare case of an elderly woman diagnosed with acute myeloid leukaemia secondary to myelodysplastic syndrome who presented a spontaneous trans-iliac bone faecal fistula probably related to an incarcerated inguinal hernia and neutropaenia. As far as we know, this is the first described case of a trans-iliac bone faecal fistula.

Objective: Fracture risk is increased in longstanding type 2 diabetes (T2D). High-resolution peripheral quantitative CT scans have demonstrated higher cortical porosity in T2D complicated by microvascular disease (MVD). We investigated if cortical bone resorption is followed by inadequate bone formation in individuals with T2D complicated by MVD.

Importance: In veterinary orthopedics, addressing foramen stenosis in the L7-S1 region in dogs presents significant challenges due to limited access and visualization with conventional surgical techniques.

Objective: This study aims to evaluate endoscopic foraminotomy through the iliac window, using three-dimensionally (3D) printing guide based on the preoperative plan, in cadaveric dogs.

Methods: Trans-iliac foraminotomies were performed on each side of five canine cadavers using and individualized 3D printing guide.

Atypical femur fractures (AFFs) are a well-established complication of long-term bisphosphonate (BP) therapy, but their pathogenesis is not fully understood. Although many patients on long-term BP therapy have severe suppression of bone turnover (SSBT), not all such patients experience AFF, even though SSBT is a major contributor to AFF. Accordingly, we evaluated tissue level properties using nano-scratch testing of trans-iliac bone biopsy specimens in 12 women (6 with and 6 without AFF matched for age and race).

Article Synopsis
  • Osteogenesis imperfecta (OI) is a genetic bone disorder that causes low bone density and frequent fractures, primarily treated with bisphosphonates in children, while adult treatment options are limited.
  • The ASTEROID trial tested a new treatment, setrusumab, which showed improvements in bone density and strength for adults with OI types I, III, and IV.
  • Analysis of bone biopsies revealed that while treatment with setrusumab did not significantly change the bone matrix properties, it did increase bone mass, suggesting that the fragility associated with OI may be due to other factors beyond just mineral content and matrix structure.

Epidemiological evidence suggests existing comorbidity between postmenopausal osteoporosis (OP) and cardiovascular disease (CVD), but identification of possible shared genes is lacking. The skeletal global transcriptomes were analyzed in trans-iliac bone biopsies (n = 84) from clinically well-characterized postmenopausal women (50 to 86 years) without clinical CVD using microchips and RNA sequencing. One thousand transcripts highly correlated with areal bone mineral density (aBMD) were further analyzed using bioinformatics, and common genes overlapping with CVD and associated biological mechanisms, pathways and functions were identified.
