Objective: The objective of this review is to gather the available evidence on the different drugs used in immune-mediated inflammatory diseases in pregnancy, lactation, their influence on female and male fertility, advice on discontinuation before conception and to help in routine clinical practice for better patient advice on family planning.
Methods: A bibliographic search was carried out, where published articles (review studies, observational studies and case series) in English or Spanish until April 2020 that analyzed the management of pregnancy, lactation and/or fertility in patients on treatment in immune-mediated diseases were selected.
Results: A total of 95 references were selected and the information on each drug was synthesized in tables.

Psoriasis is a common inflammatory skin disease that starts before the age of 40 years in 3/4 cases. Pregnancy in a couple where one of the future parents has psoriasis is therefore a frequent situation that the dermatologist may encounter and must anticipate. Contraception should be systematically discussed with psoriatic patients of childbearing age, as it is mandatory with certain treatments (as methotrexate or acitretin).

Background: This systematic review assesses effects of paternal exposure to dermatologic medications by using the former US Food and Drug Administration (FDA) pregnancy categories as a benchmark.
Objective: To assess whether systemic dermatologic medications can cause infertility and teratogenicity when taken by men.
Methods: Categories D and X dermatologic medications were identified; a systematic review of the literature and reviews of the FDA Adverse Events Reporting System and prescribing information were performed to identify the effects of these medications on male fertility and teratogenicity.
