To analyze the reliability of measurements of tarsal tunnel and medial and lateral plantar tunnel pressures before and after ultrasound-guided release. Measurements taken were guided by ultrasound to improve reliability. This novel approach may help surgeons make surgical decisions. The second objective was to confirm that decompression using ultrasound-guided surgery as previously described by the authors is technically effective, reducing pressure to the tarsal and medial and lateral plantar tunnels. The study included 23 patients with symptoms compatible with idiopathic tarsal tunnel syndrome (TTS). The first step was to measure intracompartmental pressure of the tarsal tunnel, medial plantar tunnel, and lateral plantar tunnel preoperatively. The second step was ultrasound-guided decompression of the tibial nerve and its branches. Subsequently, pressure was measured again immediately after decompression in the 3 tunnels. After surgery, the mean values significantly dropped to normal values. This represents a validation of effective decompression of the tibial nerve and its branches in TTS with ultra-minimally invasive surgery. The ultrasound-guided surgical technique to release the tibial nerve and its branches is effective, significantly reducing pressure in the tunnels and, thereby, decompressing the nerves. Level IV.

The plantaris tendon may be absent in some individuals, indicating its unclear function. Anatomically, the plantaris tendon originates from the lateral femoral condyle and has a variable course and insertion point at the calcaneal tuberosity. The plantaris tendon may influence conditions such as Achilles tendinopathy, particularly in its midportion, whether by its close relation to the calcaneal tendon or adhesions between both tendons.

Effectiveness of Using a Digital Wearable Plantar Pressure Device to Detect Muscle Fatigue: Within-Subject, Repeated Measures Experimental Design.

JMIR Hum Factors

January 2025

Department of Biomedical Engineering, Chung Yuan Christian University, No. 200, Zhongbei Road, Zhongli District, Toayuan City, 32023, Taiwan, 886 32564507.

Background: Muscle fatigue, characterized by reduced force generation during repetitive contractions, impacts older adults doing daily activities and athletes during sports activities. While various sensors detect muscle fatigue via muscle activity, biochemical markers, and kinematic parameters, a real-time wearable solution with high usability remains limited. Plantar pressure monitoring detects muscle fatigue through foot loading changes, seamlessly integrating into footwear to improve the usability and compliance for home-based monitoring.

Patients with diabetes polyneuropathy are at a heightened risk for developing foot ulcers, often due to dynamic plantar foot pressure patterns that lead to increased pressure and shear forces in specific foot areas. This study aimed to evaluate the effects of foot insoles on peak pressure and the pressure-time integral in patients with polyneuropathy diabetic foot ulcers over a twelve-week period followed by an eight-week follow up. : This was a prospective, randomized, double-blinded, controlled clinical trial involving 60 patients aged between 50 and 65 years of both genders.

Foot orthosis design for children with Charcot-Marie-Tooth and impact on gait.

Prosthet Orthot Int

January 2025

Centre de recherche du CHU Sainte-Justine, Département des sciences de l'activité physique, Faculté des sciences, Université du Québec à Montréal, Montréal, Canada.

Background: Charcot-Marie-Tooth (CMT) is a progressive disease resulting in distal sensory deficit and muscular weakness. As the disease progresses, most children develop a cavovarus foot deformity. Foot orthoses (FO) are widely prescribed in CMT to support the foot and improve gait, but there is a lack of guidelines for their conception.

Introduction: Schwannoma is a type of slow growing benign nerve sheath tumor arising from Schwann cells. Peripherally, they are found in association with the ulnar, sciatic and posterior tibial nerves. Peripheral schwannomas, neurofibromas and malignant PNSTs are collectively grouped as peripheral nerve sheath tumors.
