Published: August 2020

Study Objective: The occurrence and characteristics of contraception discussions with adolescents are unexplored. Our study sought to address this gap using transcripts of audiotaped healthcare visits.

Design, Setting, And Participants: This study was a secondary analysis of 153 transcripts of medical visits with female adolescents with asthma. Medical visits took place among 4 outpatient clinics in North Carolina.

Main Outcome Measures: Transcripts were reviewed for occurrence and characteristics of contraception discussions. Demographics were collected from adolescent interviews, caregiver questionnaires, and provider questionnaires.

Results: Contraception was mentioned in 3% (n = 5) of office visits. Conversations about contraception included the topics of contraception efficacy (20%), contraception side effects (60%), contraception adherence (20%), and adolescent sexual health (20%). No conversations included the topics of contraception indication or alternative methods.

Conclusions: Conversations about contraception occurred infrequently in healthcare visits. When conversations did occur, the topics that they covered lacked alignment with guideline recommendations.

Purpose Of Review: Polycystic Ovary Syndrome (PCOS) is a complex endocrine disorder with several causal pathways including impaired glucose tolerance, insulin resistance (IR), compensatory hyperinsulinemia and excess androgens (hyperandrogenism). This heterogeneous condition causes a range of reproductive, metabolic and psychological implications, the severity of which can differ between individuals depending on factors such as age, diet, ethnicity, genetics, medication, contraceptive use, adiposity, and Body Mass Index (BMI).

Recent Findings: Dietary interventions that focus on a low glycaemic index and glucose control are an efficient first-line dietary solution for the management of impaired glucose tolerance and IR, which subsequently improves weight management, quality of life and PCOS-related symptoms in individuals with this condition.

Bona fide diminished ovarian reserve or profound ovarian suppression by long-term oral contraceptive use?

BMJ Case Rep

January 2025

Obstetrics and Gynecology; Divison of Reproductive Endocrinology and Infertility, University of Toronto, Toronto, Ontario, Canada

Combined oral contraceptives (COCs) are widely used by reproductive-aged women. Current data suggest that long-term use of COCs can suppress ovarian reserve markers, including anti-mullerian hormone and antral follicle count, which may negatively impact ovarian response and oocyte yield in patients undergoing planned oocyte cryopreservation to preserve future reproductive potential. Discontinuation of COCs can improve ovarian stimulation outcomes, though the ideal duration of cessation is unknown.
