Recently, the (GGC) repeat expansion in the NOTCH2NLC gene has been identified to be associated with neuronal intranuclear inclusion disease (NIID). Given the clinical overlap of dementia-dominant NIID with neurodegenerative dementia, we therefore hypothesized that the NOTCH2NLC repeat expansion might also contribute to these diseases. In the present study, repeat primed polymerase chain reaction (RP-PCR) and GC-rich PCR were conducted to detect the repeats of NOTCH2NLC in a cohort of 1004 patients with neurodegenerative dementias from mainland China. As a result, 4 sporadic patients were found to carry the NOTCH2NLC repeats expansion, totally accounting for 0.4% of all dementia individuals, and the accurate repeated sizes were 110, 133,120 and 76 respectively. Of 4 mutation carriers, three and one were clinically diagnosed Alzheimer's disease (AD) and frontotemporal dementia (FTD) respectively. In addition, 3 out of them revealed leukoencephalopathy in T2-Flair imaging. This study revealed that although rare, the NOTCH2NLC repeat expansions may be associated with AD or FTD-like phenotype as well as leukoencephalopathy.

Neuronal intranuclear inclusion disease (NIID) is a rare progressive neurodegenerative disease with a characteristic pathological feature of eosinophilic hyaluronan inclusions in the nervous system and internal organs. The identification of GGC-repeat expansions in the Notch 2 N-terminal like C (NOTCH2NLC) gene facilitates the accurate diagnosis of NIID. Due to its rareness and high clinical heterogeneity, the diagnosis of NIID is often delayed or missed.

Article Synopsis
  • The NOTCH2NLC gene is linked to several neurological disorders through expansions of GGC repeats in its 5' untranslated region, which can lead to conditions like Parkinson's, dementia, and tremors.
  • The study used cellular models and CRISPR-Cas9 engineered mice with specific GGC repeat variations to investigate the effects of these repeats, particularly focusing on those with serine insertions.
  • Results showed that a specific intermediate repeat with serine insertion caused mitochondrial dysfunction and neurotoxicity, leading to PD-like symptoms in mice, including neuronal loss and behavioral impairments.

Article Synopsis
  • * An elderly male patient with sudden onset dementia was initially misdiagnosed with a vascular issue, but further tests and imaging confirmed NIID, revealing significant cortical edema and prior MRI changes in the cerebellum.
  • * The case highlights the importance of considering NIID in patients with specific imaging features and symptoms similar to other conditions like MELAS syndrome or Creutzfeldt-Jakob disease, particularly when chronic headaches and symmetric lesions in the cerebellum are present.

Background And Purpose: CGG repeat expansion in the 5' untranslated region (5'UTR) of the Notch 2 N-terminal-like C gene () has been associated with neuronal intranuclear inclusion disease (NIID) and oculopharyngodistal myopathy type 3 (OPDM3). Few OPDM3 patients have been reported. This report describes two OPDM3 patients with novel imaging findings who presented the typical features of NIID, and reviews all OPDM3 cases available in the literature.

Rationale: Neuronal intranuclear inclusion disease (NIID) is a rare neurodegenerative disorder with highly variable clinical manifestations, making diagnosis challenging. Recent advancements in genetic and pathological testing, such as the identification of GGC repeat expansions in the NOTCH2NLC gene, have improved diagnostic accuracy, but familial cases remain underreported.

Patient Concerns: This report details 3 cases of familial adult-onset NIID in 2 sisters and 1 brother.
