[Endoanchor implantation for treatment of endoleak].

The authors describe a clinical case of correction of a type 1A endoleak after EVAR using endo-anchors. An 85-year old female was referred due to an abdominal aortic aneurysm. The patient´s previous medical history included hypertension, dyslipidemia, renal insufficiency and multiple abdominal surgeries (appendicectomy, classic cholecystectomy and hysterectomy). The aneurysm had 7.5 cm diameter with an angulated short 14 mm neck extension. She underwent an uneventful EVAR with placement of a Medtronic® Endurant II stentgraft. One month after the procedure the abdominal computed tomographic angiography (CTA) revealed a type 1A endoleak. Correction of endoleak consisted of left renal artery stenting (the lowest one), placement of a proximal aortic cuff and 9 endo-anchors (APTUS® System) with satisfactory end result. The control CTA after re-intervention showed patency of the renal arteries and the stentgraft, aneurysm exclusion and absence of endoleaks.

Objective: As aneurysmal disease is progressive, proximal disease progression and para-anastomotic aneurysms are complications experienced after open infrarenal abdominal aortic aneurysm repair (AAA). As such, fenestrated or branched endovascular repair (F/BEVAR) may be indicated in these patients. Objective: The aim of this single-center case series is to demonstrate that an ultra-low dose (ULD) can be routinely achieved in the hybrid operating room in standard endovascular aortic repair (EVAR) for infrarenal abdominal aortic aneurysm by adjusting the manufacturer's predefined imaging parameters, hardware configurations and user protocols (including benchmarking).

Methods: The hybrid operating room manufacturer predefined EVAR software setup of the dose exposure control software (OPTIQ, Siemens Healthineers, Forchheim, Germany) at our university medical center was screened for possible improvements regarding radiation dose application. Tests on a water-equivalent as well as polymethyl methacrylate phantom model to assess the impact of technical settings were performed, including comparison of settings for exposure control software, different magnification, collimation configurations and detector distance.

We present a case of an 80-year-old female who presented with chest pain, vomiting and night sweats a few weeks post thoracic endovascular aortic aneurysm repair (TEVAR). A computed tomography (CT) scan demonstrated a type 1B endoleak for which she underwent a repeat TEVAR. Postoperatively, she developed fever, dysphagia, haematemesis and melaena.

Objective: We present our experience with endovascular Bentall procedure (Endo-Bentall) using a modular valve conduit (Endo-Bentall) in high-risk patients with aortic root pathologies.

Methods: The physician constructed Endo-Bentall device is composed of a self-expanding transcatheter aortic valve (TAVR), aortic endovascular stent graft (TEVAR), and two wire-reinforced fenestrations for coronary artery stenting. The TAVR valve is sutured into an appropriately sized TEVAR graft.
